Preadmission Screen and Resident Review (PASRR) Hospice Admission Procedure

Effective July 1, 2011

FOR HOSPICE ADMISSIONS ONLY

  1. A PASRR Level I screen must be done on ALL Hospice participants requiring admission to Medicaid certified nursing facilities for whom there is a certification from the client’s attending physician and/or the Hospice medical director that the client’s life expectancy is 6 months or less.
  1. Referring nursing or hospice facility staff will continue to sign, and attach to each participant’s chart, Level I screens that do not contain triggers. The nursing facility also needs to be provided a copy of the Level I screen. These screens DO NOT require submission to Ascend.
  1. For those Level I screens which have triggers, indicating a Level II evaluation might be necessary, referring staff will continue to do the Level I screen via the web only.
  1. When entering the Level I screen, with triggers, referring staff will mark all Hospice Level I screens as “Hospice” which will be selected under “payment method” on the online Level I form.
  1. A Geriatric Depression Screen (GDS) will not be required for a hospice admission. If the “Hospice” box is marked under “payment method” the screener will be able to mark “unable to obtain” when GDS Score is requested.
  1. The Level I will receive an automatic web approval through the web-based PASRR system and the participant will then be able to be admitted to a nursing facility without waiting for a Level II evaluation. Two copies of the approval should be printed out, one for the referring facility chart and one for the nursing facility. A copy of the certificate from the attending physician verifying life expectancy will be sent to the nursing facility.
  1. The Ascend web based PASRR system will provide a message to the referring staff member indicating:

This approval is good for 6 months and a Status Change Level II will be required prior to the end date of this approval, if the client is still at your facility. If a Level II is needed at that time, contact the OBRA and/or the State Developmental Disabilities Authority to arrange for it. In addition, if the Hospice client’s condition improves or they show signs or symptoms of psychiatric instability, the facility will need to contact the OBRA and/or the State Developmental DisabilitiesAuthority for a Status Change Level II.

NOTE:

Permission to admit to a nursing facility is not automatically granted in the following cases:

·  Patients admitting from a psychiatric hospital;

·  Situations where the patient has psychiatric instability;

·  If the patient is a danger to self or others.
