Dear Judge Judge's Name:

In accordance with O.C.G.A. §49-4A-9(e), the attached written report concerning the participation and progress of Youth's Name in the Department’s programs is being submitted for your review. Georgia law requires the Department to notify the court that further disposition is necessary. The law further provides that the committing court shall review Youth's Name’s case prior to his or her seventeenth birthday. The court is to determine whether Youth's Name, upon becoming seventeen years of age, should be placed on probation, have his/her sentence reduced, be transferred to the Department of Corrections for the remainder of the original sentence, or be subject to any other determination authorized by law.
