LAKEVIEW LOCAL SCHOLARSHIPS

The following scholarship applications will be available on Monday, March 4, 2013 and must be completed and returned to the Guidance Office by THURSDAY, April 11, 2013 by 2:30 PM. LATE OR INCOMPLETE APPLICATIONS WILL NOT BE ACCEPTED! You must provide your own activity sheet; no copies will be made for you.

ALEXANDER ELONEN, JR. MEMORIAL SCHOLARSHIP:

This scholarship is awarded to a Lakeview High School senior who will major in the field of engineering to include but not limited to: aerospace, mechanical, medical, electrical/electronic, industrial, chemical, civil, or nuclear, at an accredited post-secondary institution.

BULLDOGS TO BUCKEYES SCHOLARSHIP: This $500 scholarship is reserved for Lakeview seniors who will be attending The Ohio State University. An essay and financial need will be factors in awarding this scholarship. Proof of enrollment at OSU will be required.

CAROL CALLAHAN SNODGRASS MEMORIAL SCHOLARSHIP: This scholarship is provided by the Snodgrass family and friends and will be awarded to the senior cheerleader who does the most to support the Lakeview “Blue and White.”

***COCA-COLA SCHOLARSHIP: Scholarship will be based on academic achievement and financial need. The Student Aid Report from your FAFSA is required. Two recommendations are required. *The Roush, Coca-Cola, and Medical Mutual of Ohio scholarships will use a combined application.

CODY BETTS MEMORIAL SCHOLARSHIP: This scholarship will be given to a boys’ basketball player who has shown a high degree of commitment to Lakeview High School.

CORTLAND–BAZETTA FRATERNAL ORDER OF POLICE: These scholarships are awarded to students pursuing a career in the field of law enforcement. Scholarships will first be earmarked for students entering an Ohio police academy program only; students pursuing a college degree in criminal justice may also apply.

CORTLAND FIREFIGHTERS ASSOCIATION: This scholarship is provided by the Cortland Firefighters Association and is available to a senior based on his/her community service, a minimum 2.0 GPA, and required essay.

DR. BEDICH HEALTH PROFESSIONS: This scholarship is provided by Dr. Bedich and is available to any senior with at least a 3.2 cumulative GPA and who will be majoring in a health-related field. An essay is required.

DRS. DAVENPORT & ROHRABAUGH: This scholarship is provided by Dr. Davenport and Dr. Rohrabaugh and is based on acceptance to a college or university majoring in the health or science field. Financial need and a short essay are also factors.

JAIME L. SCOTT MEMORIAL SCHOLARSHIP: This scholarship is provided by the Scott family and is available to a student with special needs who has an I.E.P. and is based on academic achievement, attendance, and future potential.

LAKEVIEW ALUMNI MUSIC SCHOLARSHIP: One music scholarship of up to $500 may be awarded to a graduating senior from Lakeview High School. The candidate must be planning to major in music at a collegiate school of music in the fall following graduation from Lakeview High School.

LAKEVIEW BOARD OF EDUCATION: The members of the Lakeview Board of Education offer a scholarship to a senior who has demonstrated exemplary community service. An essay is required.

LAKEVIEW BULLDOG BOOSTERS (PAT BERRY) MEMORIAL SCHOLARSHIP: This scholarship is presented by the Bulldog Boosters, and is available to one male and one female athlete who is a three-year varsity letter winner in one sport or a two-year varsity letter winner in two sports. The winner must also have a 2.5 GPA, two letters of recommendation, and a member of the student’s family must be a paid member of the Bulldog Boosters club.

LAKEVIEW CLASS OF 1977 ALUMNI SCHOLARSHIP: One $500 scholarship will be awarded based on the following criteria: academic achievement, extra-curricular participation, community service, and a documented intention on attending a local post-secondary school.

LAKEVIEW LITTLE BULLDOGS: This scholarship is provided by the Lakeview Little Bulldogs Booster Club and is available to any senior who has participated in their program as a player, cheerleader, pom-pom girl or in some other capacity.

LAKEVIEW MIDDLE SCHOOL PTO: This scholarship is provided by the Lakeview Middle School PTO and is based on extra-curricular activities, community service, and financial need. One male and one female will be selected.

LAKEVIEW OUTREACH AND FELLOWSHIP (REV. STEPHEN STOUT) MEMORIAL SCHOLARSHIP: Any student attending a L.O.A.F.-member church and who has an intention of pursuing a career in the ministry field is eligible.

LAKEVIEW TEACHERS’ ASSOCIATION SCHOLARSHIP: This scholarship is provided by the Lakeview Teachers’ Association and is open to all Lakeview seniors. However, preference is given first to those seniors who have parents that are current members of the LTA, and second, to those majoring in education.

LIONS CLUB: HONORS scholarships will be given to a Lakeview Honors student (3.5 GPA or higher) and FINANCIAL NEED scholarships will also be given based on financial need. W-2 forms are required for financial need applicants.

LT. COL. ROBERT STOCKTON MEMORIAL SCHOLARSHIP: Preference given to students entering the military service after high school. Air Force is preferred, but all branches will be considered.

MATT SPENCER MEMORIAL SCHOLARSHIP: This scholarship is provided by the Spencer family and is available to any senior continuing their education. An essay is required on the topic of friendship.

***MEDICAL MUTUAL OF OHIO: This scholarship will be based on academic achievement and financial need as determined by the Student Aid Report and FAFSA. *The Roush, Coca-Cola, and Medical Mutual of Ohio scholarships will use a combined application.

OPTIMIST CLUB: Scholarships are based on service to the school and the community. Two recommendations are required.

RICHARD WILDER MEMORIAL SCHOLARSHIP: This scholarship is intended for any high school graduate who has suffered the loss of either one or both parents due to any circumstance or has a severely disabled parent. This scholarship is meant to help the surviving child with college expenses.

ROTARY CLUB: Scholarship criteria include service to community, academic achievement, character, financial need, and involvement in Rotary projects. Two recommendations are required.

SUELLEN GOLUBIC MEMORIAL SCHOLARSHIP: This scholarship is provided by the Golubic family and friends. This scholarship is available to any Lakeview senior that will be attending a post-secondary school and has maintained a minimum 2.5 GPA. However, preference will be given to students majoring in education and health professions.

TAMI HARTER MEMORIAL SCHOLARSHIP: This scholarship is provided by the family and is available to a female student pursuing pharmacy or a health-care profession. Other considerations will include active participation in sports, school activities, a minimum 3.5 GPA and community service.

THOMAS H. SUDIMACK MEMORIAL SCHOLARSHIP: This scholarship is provided by the Sudimack family and is available to a varsity athlete who has a minimum 2.5 GPA.

TRACEY L. HUSK MEMORIAL SCHOLARSHIP: This scholarship is provided by the friends and family of Tracey Husk, a 16-year veteran of Lakeview schools who taught in the 4th grade gifted program. Tracey died from cancer in September 2009. Applicants who are pursuing a career in elementary, middle school, or gifted education, have at least a 3.0 GPA, and who have participated in community activities will be considered.

ULISSE DELPIERO MEMORIAL SCHOLARSHIP: This scholarship is awarded to a student pursuing a career in the field of law enforcement. Preference is given to those students who would like to serve as a Cortland police officer. Students pursuing a college degree in criminal justice may also apply.

***WILLIAM T. ROUSH MEMORIAL SCHOLARSHIP: Scholarship will be based on financial need and academic achievement. The Student Aid Report from your FAFSA is required. Two recommendations are required. *The Roush, Coca-Cola, and Medical Mutual of Ohio scholarships will use a combined application.
