*Parishioners are registered members of a Parish, who are actively living their faith by attending weekend Mass and by the stewardship of time, talent, and sharing of the treasure. At what Catholic Church are you registered and actively living your faith?______

Tuition for the 2017-2018 school year will be paid by: (Circle one Option)

OPTION 1: Single Payment due August 16, 2017. Payment is made directly to the school.

OPTION 2: Two Payment Plan: Two equal payments are made directly to the school.

1st payment due August 16, 2017 2nd payment due January 16, 2018

OPTION 3: FACTS monthly payment plan - payments are debited from your bank account over 10 months beginning July 2017 through April 2018. Payments may be made either on the 5th or 20th of the month. A $43 fee will be charged to each family. This is a FACTS charge imposed on the school for this service.

Note: A $30.00 charge will be assessed by FACTS each time a payment is attempted, but

funds are not available.

OPTION 4: Four Payment Plan: Four equal payments are made directly to the school.

1st payment due August 16, 2017 2nd payment due October 16, 2017

3rd payment due January 16, 2017 4th payment due April 16, 2018

Note: A $25.00 charge will be assessed monthly for late payment of Option 1, 2 and 4.

If you choose to pay monthly through FACTS, you will need to fill out online a FACTS Automatic Tuition Payment Plan Agreement Form at www.online.factsmgt.com. The form needs to be completed by April 15th. If you have questions, please contact the school office at 529-4511.

I agree to make tuition payments for the 2017-2018 school year according to one of the options above. Failure to make timely tuition payments will lead to collection efforts which may include withholding report card(s), possible dismissal from school or referring account to a collection agency.

Annual tuition assistance applications are available online at www.online.factsmgt.com/signin and need to be submitted to FACTS by April 30, 2017. A $30.00 non-refundable application fee is charged by FACTS. This application is only good for school year 2017-2018.
