L2 Visa Application

Outlined below, you will find a list of documents required for your dependent (spouse/child) to obtain an L2 visa stamp at a U.S. Embassy abroad.

1. Your original L1 approval notice (form I797) issued by INS or a copy of the blanket L1 approval notice (form I797) issued to your company;

2. An attorney certified copy of the L1 petition filed with INS on your behalf OR a copy of the L1 visa application used to apply for your L1 visa stamp if you entered the U.S. under your company’s blanket L program

3. Complete copy of your passport

4. Copy of your most recent I94 card issued at the U.S. port of entry

5. Copy of your marriage certificate

6. Copy of your dependent’s birth certificate

7. Dependent’s passport valid for 6 months beyond the expiration of your L1 status

8. Completed form DS-156 (visa application form)

9. One passport size photograph of your dependent

10. OPTIONAL: Affidavit of support (form I134) or letter confirming employment with your current employer which lists salary or a copy of a recent paycheck stub from your employer

We also recommend that you call the Embassy ahead of time to ensure that your dependent has the correct documentation to ensure issuance of the visa and to determine the Embassy’s hours of operation.

We would appreciate you forwarding a copy of the new visa stamp and I94 card (front and back) to our office when your spouse/child enters the U.S.
