Student Safety Contract

I, ________________________________recognize that the science laboratory will be a safe place in which to work and learn if I conduct myself in a responsible manner at all times. I agree to assume responsibility for my own safety and for the safety of my classmates. I agree to follow the safety guidelines set forth in the laboratory procedures and to follow the teacher’s instructions at all times. In addition, I will abide by the following safety rules.

Ø I will not bring food and /or beverage into the laboratory area.

Ø I will wear appropriate clothing on lab days

Ø I will wear safety goggles when required

Ø I will handle all laboratory equipment properly

Ø I will familiarize myself with laboratory techniques involved in each activity before I attempt to perform the activity.

Ø I will learn the location and proper use of the safety equipment and first aid in the laboratory.

Ø I will report any accident to the teacher immediately and know where to get help if needed.

Ø I will familiarize myself with the procedures to be followed in case of fire in the laboratory.

Ø I will keep my workstation clean and organized.

Ø After each activity has been completed, I will dispose of all chemicals or specimens according to teacher’s direction and I will return all materials and equipment to their proper place.

Ø I will follow any additional instructions, written and /or verbal, provided by the teacher.

I understand that I will not be allowed to participate in the laboratory activity and could possibly receive a grade of zero if I do not follow the safety rules above.

Course Policy, Lab-safety, and equipment-use agreement

I have read and discussed the safety rules outlined above, the appropriate computer rules outline in the student handbook, and the equipment use expectations on the reverse side of this sheet with my student. My student and I understand that violation of these rules may put him/her and other students in harm’s way. For this reason disciplinary action to include suspension of lab privileges could be taken.

My student and I understand that accountability is an important part of any process especially the study of scientific process. The equipment, textbooks, and computers must be used with care so that they will be available to other students for years to come. I understand that payment will be required for any equipment my student breaks due to inattentiveness or incorrect or inappropriate use.

I have read and discussed with my student the information found on the course policy sheet.

Appropriate Use of Science Equipment

Science equipment whether it is new or old is expensive and difficult to replace. We need students to help us take care of our facilities and equipment.

Students usually work in groups of two, three, or four depending on space and equipment availability. Equipment in good condition will be loaned to the student groups for use during laboratory activities. Some activities require the use of computers, probes, and software packages. Some supplies are consumed as part of the activity. However all other equipment must be used with care as described in either written or verbal instructions. It is expected that the equipment will be returned in the same condition it was issued. If the equipment is broken due to student negligence, such as not following directions, unauthorized experimentation, inattentiveness or goofing around the student will be required to pay the replacement cost of the equipment. Vandalism will be referred to an administrator for disciplinary action in addition to the requirement for payment of the vandalized equipment.

I agree to respect the teacher and my classmates by not interfering with the learning process. One way to do this is to use the equipment with care. I understand that the equipment is loaned to me and must be returned in good condition. I agree to folllow the written directions and the teacher’s verbal instructions as to the correct use of the equipment. I understand that payment wil be required for any equipment I break due to inattentiveness or incorrect or inappropriate use.
