JOB TITLE: Permanent Supportive Housing (PSH) Case Manager

REPORTS TO: Coordinator of Housing

EMPLOYMENT STATUS: Non-Exempt

TIME COMMITMENT: Full-time

STARTING SALARY: Commensurate with Experience

Position Summary

PSH Case Manager oversees Catholic Charities’ scattered site supportive housing program in the community, focusing on case management of the chronically homeless permanent supportive housing program. Embodying the standards as a HUD-Certified Housing Counseling Agency, this Case Manager is responsible for implementation and oversight of landlord relationships, lease programs, creation of a home-share program; provision of case management for those living in permanent supportive housing, finding and retaining housing, plus identifying sources for housing funding, housing-based education, credit counseling and linkage to appropriate related community resources.

Essential Functions

1.  Implement a strong case management component for the chronically homeless and services focused on moving people out of homelessness quickly as well as preventing homelessness recidivism.

2.  Ensure compliance with case management standards as delineated in HUD’s Housing Counseling Handbook. Case management and recordkeeping includes, but is not limited to: intake; service planning; service delivery including advocating for clients with property owners/managers as necessary and providing resource information; and case closure.

3.  Maintain a broad-based working knowledge of HUD, its Housing Counseling Handbook and local housing programs, and ensure staff/volunteers are well-versed and current in their knowledge.

4.  Identify and develop program-specific outcomes to ensure program effectiveness and impact, and the ability to report that to funders and the community. Design and manage program services directly related to those outcomes.

5.  Develop and manage a comprehensive outreach/ working relationship with stakeholders including other community members, especially landlords. Implement a landlord recruitment and retention plan. Maintain awareness of changes in market and community processes that can impact services; bring forward prospective recommendations to deal with expected changes.

6.  Develop and implement permanent supportive housing program for chronically homeless in alignment with Housing First model.

7.  Design and oversee new home-share program for Catholic Charities’ shelter and housing participants.

8.  Coordinate case management with other human service providers assisting clients.

9.  Develop new program components as directed and provide on-going education classes on related housing topics.

10.  Maintain up-to-date client files and program statistics, including, but not limited to, client data, volunteer service sheets, and grant-related documentation.

11.  Maintain materials, equipment, and forms necessary for program operations.

12.  Perform home visits and off-site case management meetings.

13.  Assist in facilitating the program to be in compliance with Council on Accreditation (COA) standards. Be thoroughly familiar with the policies and procedures guiding the work of this position in particular and the program or department overall. Perform job functions in line with these policies and procedures.

14.  Participate in training required by accreditation standards, plus performance and quality improvement efforts.

15.  NOTE: Catholic Charities considers this position to be a mandated reporter of elder and child abuse.

Education, Experience & Skills Required

1.  A bachelor’s degree and at least two years of experience in human services preferred.

2.  Three to five years of experience in human services, including at least two years of experience with shelter or outreach services required.

3.  Strong written and verbal communication skills; organizational, conflict resolution and computer literacy.

4.  Two to three years of experience that reflects an understanding of causes of homelessness.

5.  Two plus years of experience working with landlords, local rental markets and/or housing sales.

6.  Knowledge of the Housing First program(s).

7.  Cooperative, friendly, and helpful attitude with clients and co-workers. Ability to work closely with other employees to ensure positive, constructive environment within the program or department, and throughout the agency.

8.  Ability to be flexible, adapt and adjust to rapid changes within a fast paced and growth-oriented environment. Possess and maintain a positive, solution-oriented approach.

9.  Passion and enthusiasm for the mission of CCDSR and its clients.

10.  Possess valid driver's license; at least state required minimum of auto insurance.

11.  Computer literacy required, including experience with Excel, Outlook, Word, etc.

12.  Bilingual (English/Spanish) preferred.

13.  Must pass fingerprinting prior to start of employment.

14.  Demonstrated experience in working with the homeless or low income individuals and families. Sensitivity to issues surrounding individuals and families experiencing homelessness, substance abuse, mental illness, or physical ailments.

15.  Ability to work under pressure. Experience in crisis intervention and problem solving with ability to diffuse situation without heightening the conflict.

16.  Exhibit the core competencies, habits, critical thinking, attitudes, behavior, and drive to help make Catholic Charities and its programs a center of excellence and a model program.

Physical Requirements

1. Requires ability to sit up to 3-6 hours per day with intermittent occasional walking and standing.

2. Occasionally may be required to lift items up to 10 pounds to a height up to 7 feet.

3. Occasionally may be required to carry items up to 10 pounds for distances up to one block.
