PETITION TO WAIVE ANNUAL ACCOUNTINGS

Petitioner, _____________________________, Guardian of the Property of ______________________________, Ward, petitions the Court to waive annual accountings on the basis that: (check all that apply)

( ) The Ward receives income only from Social security benefits and the guardian advocate is the person’s representative payee for the benefits

( ) There is no property in the guardianship estate. (Attach proof of bank statements).

Petitioner understands that if the ward receives property in the future, the Guardian of the Property shall file a Verified Inventory within fifteen (15) days of receipt of the property.

Wherefore, Petitioner requests the Court waive annual accountings.
