COMMUNITY SERVICE FUND (GRANT)

APPLICATION INSTRUCTIONS

AND GUIDELINES

Idaho Waste Systems, Inc. has established a Community Service Fund (Grant) for Elmore County Projects. Matching funds are required by an equal cash amount for any qualified and selected community service projects.

The grant is subject to the following conditions:

In order to qualify as a community service project, the said project must involve capital improvements to new or existing community service facilities, for example: Construction of community swimming pools, community parks, community center rehabilitation and development. These monies may be used for any other community service projects or programs based upon a unanimous vote of a committee appointed to oversee the selection of any qualifying community service projects. The Committee may upon unanimous vote consider other community service projects.

The project needs to be started within one (1) year from the time the grant is awarded. An estimated completion date is required and quarterly updates on the project will be turned in to the Clerk’s Office. If for any reason the project is not started, the Committee may award the grant as they choose to another applicant. Funds will be distributed on a monthly basis after presentation and review of vouchers. Vouchers must be signed by the Chairman or President of the organization the grant was awarded too. Any person who knowingly makes any false statement to obtain a grant or knowingly files a false document or makes a false statement in the administration of the grant may be in violation of Idaho law and subject to prosecution.

Please print and complete the attached application with the required documentation, and return to the Elmore County Clerk’s Office, Elmore County Courthouse, 150 South 4th East, Suite 3, Mountain Home, Idaho, 83647 no later than April 24, 2015.
