ICO Fellowship Program Application Checklist
Look through “Directory of Training Centers”
Decide on subspecialty to study
Choose host in a foreign country
Send host your CV
Once host accepts, tell ICO within 7 days
Choose fellowship timing with host
Send Form A to ICO by email
Fill out Form C with host
Download and fill out application using Microsoft Word
Print out application form when finished
Sign application form
Attach any necessary documents (i.e. letter of recommendation)
Scan application form and documents onto your computer
Save documents in .jpg, .gif, or .pdf format
Make sure files will be printable in Din-A4 format
Take scanned files and compress into 1 zipped (compressed) file
Email zipped file to ICO:
