Heritage Middle School
Physical Education
Requirements, Expectations, and Grading Procedures
Required Materials and Equipment:
1. Plain, white, short sleeved T- shirt. Students’ name must be printed on the front neatly. *
2. Solid Navy blue shorts with no logos. Students’ name must be printed neatly on the left leg of the short. Teachers will have a silver Sharpie. Length must be Dress Code length. *
3. White socks.
4. Athletic type tennis shoes with arch support. Example: Keds and Vans are not athletic shoes.
5. Hygiene products. Example: deodorant (no cans of spray or glass containers).
6. Folder with brads for study guides, tests, etc.
7. Hair tie and/or clips.
8. No jewelry may be worn during class. Bring a zip lock baggy to keep jewelry from being lost.
* Students have the opportunity to purchase an HMS Physical Education uniform (T-shirt and shorts). An order form will be sent home if you did not order when you picked up your schedule. Friday, August 29th is the last day to order for the next shipment. More orders will be taken after Labor Day.
Care of Equipment:
Each student is responsible for keeping up with his/her uniform and lock. Students will be reminded to take their uniform home at least once a week to clean.
If a lock is lost or damaged, there is a $10 fee to replace the lock. A new lock will not be issued until the fee is paid. Parents will be notified once a week by the teacher if a lock is missing.
Be sure that the locker is closed properly and the lock is locked at all times. Do not leave clothing or valuables outside of your locker at any time. The student is responsible for any missing items.
Dressing Out:
1. Students must suit out in the proper uniform as previously described.
2. Students that do not have their uniform will be issued “loaner” clothes for that period. Points will be deducted from their grade. The first time is considered a free warning, second time the student will call the parents, and the third time will result in a detention.
3. Proper footwear must be worn daily for safety precautions. Students without proper footwear must still dress out but will not be allowed to participate in the day’s activities. Additionally, a grade of zero will result for the day.
4. Jewelry is not allowed in physical education class for safety reasons.
5. Hair must also be tied back for safety reasons.
6. Socks must be worn for hygiene reasons.
Excuses for Limited Participation:
Parental notes can excuse a student from certain participation in the daily activity for up to two days. The student is still required to suit out in proper attire and participate in stretching activities and any other activities not affected by the ailment. Any restrictions lasting longer than two days must be accompanied by a doctor’s note. A doctor’s note must include the following:
1. The injury sustained or the illness.
2. Specific restrictions (i.e. no push-ups).
3. Duration of restrictions.
Absences:
Students who have an excused absence will need to ask their teacher for any make-up work upon returning to school.
Locks and Lockers:
1. A combination lock will be issued and a gym locker will be assigned. Lost or damaged locks will result in a fine of $10.00.
2. It is your responsibility to notify your teacher if you are missing a lock. Parents will be notified in writing once a week of any missing locks.
3. Students may not switch lockers once assigned and they may not share a locker with anyone.
4. When placing your uniform in your locker, close the door, latch the lock through the holes, and turn the dial. If something is missing, look in the lost and found box in the locker room first. There will be consequences if you do not lock your belongings in your locker.
5. Locks and lockers have all been repaired or replaced during the summer.
6. Books and binders are not allowed in the gym or locker room. They cannot be secured in your P.E. locker and it is a safety hazard to have them on the floor in the gym.
FitnessGram:
FitnessGram is a health related fitness assessment. It will assess cardiovascular endurance (one mile run), flexibility (sit and reach), upper body strength (modified pull-ups), abdominal strength (curl ups), and body composition. This will be administered at the beginning of the year and toward the end of the school year. A FitnessGram report will be sent home at the end of the year to show improvement.
Classroom Programs during Physical Education:
Physcial Education classes will meet in classroom 605 on a weekly basis. Parents are encouraged to attend the district’s Parent Preview meetings of the curriculum. Please see the attached letter from the district coordinator.
All 6th grade Physical Education students will participate in D.A.R.E. Officer Wheeler will conduct this 10-lesson program.
The district’s Abstinence Program, Choosing the Best, is 6 lessons for both 6th and 7th graders. A permission letter is being sent home in addition to this packet. Prior to this unit, an additional permission form will be sent home as well and can also be accessed on the district website under the health curriculum programs. You do have the option of allowing your student to participate in this unit or in an alternative activity.
Other health lessons including human reproduction, nutrition, and character pillars (i.e. respect, honesty, caring, etc.) will be taught as well.
Grading Standards:
1. Participation accounts for 50% of the total six weeks grade. The student participates in all class activities on a regular basis. This includes being on time and prepared for class with the proper materials. Any homework that is assigned is included in this category.
2. Effort accounts for 50% of the total six week grade. Behaviors students should exhibit are hustle, performing activities with more than minimal effort, and consistently listening to and following instructions.
