GDS ST GOALS

Stage 3

1. Patient will demonstrate the use of calendars, schedules, and activity calendars with 90% accuracy given minimal verbal and visual cues to improve orientation and increase participation in daily activities.

2. Patient will verbally request clarification of directions, questions, or conversations in 9/10 opportunities to enhance conversation with communication partners.

3. When given verbal directions, patient will repeat sentence or sentences in 8/10 opportunities to enhance comprehension of complex tasks.

4. Patient will demonstrate the use of printed materials (labels, directions, signs) to complete activities of daily living with 90% accuracy given min cues, to increase independence in current living environment.

5. Patient will answer open ended questions with 90% accuracy given min cues, in order to express wants and needs.

6. Patient will demonstrate the use of circumlocution during conversational tasks in 8/10 opportunities given min cues, to enhance communication.

7. Patient will complete financial and medication management tasks with 90% accuracy given min cues to enhance independence in environment.

8. Patient will use verbal problem solving with 90% accuracy given min cues to improve safety and awareness in environment.

9. Patient will sequence ADL’s with 90% accuracy given min cues to increase independence and safety.

10. Patient will demonstrate use of appropriate conversational skills with 90% accuracy given min cues to increase patient’s involvement in social situations. Skills to include: conversational turn-taking, participate in social routines, and verbal and non-verbal communication.

Stage 4

1. Patient will sequence pictures or printed materials with 90% accuracy given min cues to enhance safety and independence.

2. Patient will demonstrate ability to navigate living environment with 80% accuracy given minimal verbal and visual cues.

3. Patient will use daily schedule with 90% accuracy given moderate cues to aid in patient’s independence.

4. Patient will follow multi-step printed or verbal directions with 80% accuracy given min cues to increase independence in activities of daily living.

5. Patient will initiate conversation in 8/10 opportunities to enhance social conversation.

6. Patient will demonstrate orientation to person, place and date using verbal and visual aids with 80% accuracy given mod cues to aid in decreased confusion.

7. Patient will create/use a memory book/aid to participate in conversational tasks in 8/10 opportunities to aid in memory recall.

8. Patient will visual scan printed materials in functional tasks with 80% accuracy given min cues to increase functional independence in current environment.

9. Patient will request repetition of question/statement in 8/10 opportunities given min cues to increase social interaction and socialization.

10. Patient will complete verbal problem solving tasks with 80% accuracy given min cues to maximize safety in living environment.

Stage 5

1. Patient will demonstrate orientation to person, place and date using verbal and visual aids with 80% accuracy given mod cues to aid in decreased confusion.

2. Patient will increase attention to conversational task or activity to > 10 min in 8/10 opportunities given min verbal and visual cues.

3. Patient will answer yes/no questions with 90% accuracy to increase ability to express wants and needs.

4. Patient will use memory/book aid to participate in conversation tasks with familiar and unfamiliar partners in 8/10 opportunities to increase recall of familiar events and people.

5. Patient will follow 1 and 2 step directions with 90% accuracy given min/mod verbal and visual cues to increase independence in functional tasks.

6. Patient will sequence ADL’s with 80% accuracy given mod cues to increase independence.

7. Patient will demonstrate no unsafe behaviors in 8/10 opportunities given verbal cues to increase safety in living environment.

8. Patient will answer open ended questions with 80% accuracy given min cues to communicate wants and needs.

9. Patient will participate in group activities with appropriate stage partner in 8/10 opportunities to increase patient’s socialization.

10. Patient will perform categorization and organization tasks with 90% accuracy given mod cues to improve problem solving.

Stage 6

1. Patient will sequence steps of daily activities during functional tasks (bathing, eating, dressing) with 90% accuracy given mod cues to enhance in independence.

2. Patient will increase attention to conversational task or activity to > 5 min in 8/10 opportunities given mod verbal and visual cues.

3. Patient will use simple words/phrases and gestures to communicate in 8/10 opportunities given mod verbal and visual cues to communicate wants and needs.

4. Patient will answer yes/no questions with 80% accuracy given mod cues to communicate wants and needs.

5. Patient will make choices from a field of two with 90% accuracy given mod cues to enhance patient’s ability to express wants and needs.

6. Patient will increase PO intake to >50% during meal times given min cues to maximize nutrition and hydration. Cues include: verbal, visual and tactile cues, environmental modification, and/or staff education

7. Patient will initiate and attend to self feeding with 80% accuracy given mod cues to increase independence and nutrition/hydration.

8. Patient will demonstrate no unsafe behaviors in 8/10 opportunities given mod cues. Cues include: redirection, patient and staff education, written cues

9. Patient will demonstrate decreased anxiety or agitation in 8/10 opportunities following caregivers/staff use of redirections, positive gestures, and calm tone of voice.

10. Patient will participate in safe swallow strategies with 80% accuracy given mod cues to decrease risk of aspiration/penetration.

Stage 7

1. Patient will increase PO intake to >50% during meal times given mod cues to increase nutrition and hydration. Cues include: verbal, visual and tactile cues, redirection, environmental modification, and/or staff education.

2. Patient will participate in safe swallow tasks with 90% accuracy given max cues to decrease risk of aspiration/penetration and increase hydration and nutrition.

3. Patient will make choices from a field of two with 90% accuracy given mod cues to enhance patient ability to express wants and needs.

4. Education provided for caregivers to reduce distractions during functional tasks with 80% accuracy to increase task maintenance.

5. Patient will demonstrate decreased anxiety or agitation in 7/10 opportunities following caregivers/staff use of redirections, positive gestures, and calm tone of voice.

6. Patient will decrease initiation of swallow to a timely manner in 7/10 opportunities given mod verbal, visual, and tactile cues.

7. Patient will accept presentation of food and drink with 80% accuracy given max verbal, visual, and tactile cues.
