Yes! I, _________________________, wish to accept the opportunity to enroll in SUNY Canton for the fall 2016 semester, and participate in the GATEWAY TO SUCCESS PROGRAM. While I understand the purpose of this program is to help improve my chances for academic success and enrollment into my intended major, I fully understand that I am ultimately responsible for my own academic success. As such, I intend to meet all the program requirements in order to fully gain the benefits of the G2S PROGRAM. I understand that enrolling in the program is a semester commitment and I cannot opt out. I also understand that completion of all assigned courses with a “C” or better will be necessary to enter my major of choice. Participation in Gateway to Success ALONE, does not guarantee you will be admitted to any particular major.

_____ Attend the informational meeting on Wednesday, August 31st at 5:30 pm in Miller Campus 218-220.
You CANNOT skip this meeting.
_____ Complete the Academic Success Plan and meet with my G2S mentor to discuss the plan by
FRIDAY, SEPTEMBER 9TH. (I understand it is my responsibility to initiate scheduling the meeting with my Gateway mentor.)
_____ Attend ALL class meetings on time, and fulfill ALL class requirements and participation expectations.
_____ Attend/sign into a tutoring lab for a minimum of three (3) hours per week.
_____ Attend at least one session of the Smart Steps workshops (schedule provided in this package.)
_____ Attend ALL pre-scheduled meetings with my Gateway mentor and any additional meetings requested by my
mentor. (I understand it is my responsibility to check my Canton email daily for important notifications from my mentor)
