Congratulations! You’ve made a very sound decision to implement consistent assignments at your facility and the CAHF QI Subcommittee wants to assist you in this exciting endeavor. Attached please find a “toolkit” of resources designed to assist leaders and individuals to effectively implement consistent assignments in their facility. Subcommittee members have developed some of these materials, others have been provided by the Quality Improvement Organizations and other sources. All materials are designed to guide you through the process and gain the benefits of consistent assignments as quickly and smoothly as possible. Included in this toolkit are the following:
- Letter to Administrators, Directors of Nursing and Directors of Staff Development
A one-page document describing the benefits of consistent assignment and providing a compelling rationale for making the change
- Letter to CNAs
A one-page document intended to be distributed to all CNAs with their paycheck or via other means, explaining your decision for transitioning to consistent assignments and describing the benefits
- PowerPoint for Administrative and Management Team Member
A presentation that describes the various literature supporting the rationale and benefits of consistent assignment
- Step-by Step Approach for Making the Change to Consistent Assignment
Consisting of two parts, this section includes an “At-a-Glance” implementation page and a more comprehensive change ideas document, both of which provide step-by-step implementation ideas for making the change to consistent assignment
- Calculation of CNA Turnover

- Consistent Assignment Brochure/Poster
Document that can be used as a brochure to educate families and friends about consistent assignments, or as a poster, providing recognition to staff implementing consistent assignment (versions in color or black and white)
