-- Providing the Child with a Summary of Academic Achievement

and Functional Performance

Section 614 (c) (5) (B) (ii)

For a child whose eligibility terminates due to graduation from secondary school with a regular diploma, or due to exceeding the age eligibility for a free appropriate public education, the school district shall provide the child with a summary of the child’s academic achievement and functional performance, which shall include recommendations on how to assist the child in meeting the child’s postsecondary goals.

What is a “summary of academic achievement and functional performance?”

§ IDEA 04 also requires that the child’s Individual Education Program (IEP) include a statement of the child’s present levels of academic achievement and functional performance. So part of the summary would be the IEP’s Present Levels statement. In Kentucky, ARCs also consider the Individual Graduation Plan (IGP) to be reflective of the child’s high school assessments, activities, course of study, and academic progress. So part of the summary would be the child’s completed IGP.

Other information would be also included in the summary and would relate to the child’s postsecondary goals. For example, a child who plans to enter a postsecondary education institution would also need a copy of transcripts. A child who plans to seek competitive employment would also need a copy of a resume reflecting past work experiences.

What are “recommendations on how to assist the child in meeting the postsecondary goals?”

§ The summary of academic achievement and functional performance should include information that will directly assist the child in meeting the child’s postsecondary goals. This may include additional information provided to the child or needed interagency linkages outlined for the child. For example, if a child’s postsecondary goal is to enter a college or university, the summary should reflect a linkage with the Disability Services Coordinator and necessary documentation of disability should be provided to the child or arrangements made for the child to obtain the necessary documentation to ensure a smooth transition to postsecondary education. If a child’s postsecondary goal is to be competitively employed, a linkage should be made with the Office of Vocational Rehabilitation and if possible, a referral and eligibility established before graduation. Many students are still unsure of their postsecondary goals and so recommendations must be more general. For example a resource listing of available adult service providers may be given and explained to the child.

SUMMARY OF ACADEMIC ACHIEVEMENT AND FUNCTIONAL PERFORMANCE

The following guide can be used to assist districts in meeting IDEA 2004 requirements for students whose special education eligibility terminates due to graduation with a regular diploma or exceeding the age eligibility requirements. Under these provisions, the district shall provide the student with a summary of the child’s academic achievement and functional performance, which shall include recommendations on how to assist the child in meeting the child’s postsecondary goals.

The Summary of Performance must be developed during the final year of a student’s high school education. The timing of completion of the Summary of Performance may vary depending on the student’s postsecondary goals. If a student is transitioning to higher education, the Summary of Performance, with additional documentation, may be necessary as the student applies to a college or university. Likewise, this information may be necessary as a student applies for services from state agencies such as vocational rehabilitation. It is most appropriate to wait until the spring of a student’s final year to provide the student with the completed documentation. The Summary of Performance may be provided to the student during an exit/transition review which can be conducted in conjunction with the transition session of the Individual Graduation Plan process or in conjunction with the spring Admissions and Release Committee annual review meeting.

Specific disability agency contact information, describe An appointment was made for Anita to meet with a Benefits Plus Coordinator: Anita was given the name and number of her Office of Vocational Rehabilitation Counselor and her Supported Employment Counselor. She was also given the name and number of her Case Manager from Community Mental Health.

C. Additional Comments, as appropriate:

Anita’s wants to work in a daycare or childcare facility. She has earned a diploma and has had coursework in human development. Anita did well in these classes with accommodations. (See transcripts). Anita also had IEP goals and objectives related to self-determination and work skills. Her progress data shows continuous growth in these areas. Anita’s resume outlines her work experiences and career exploration experiences in the area of childcare. She has several recommendation letters attached to her resume. (See resume and IGP). Anita has been working with the Office of Vocational Rehabilitation to begin her involvement with Supported Employment. The Supported Employment Counselor has planned a first placement for work for Anita upon graduation. Anita has also been referred for services with the Community Mental Health agency specifically for assistance with independent living. A case manager is scheduled to contact Anita upon graduation.

Anita works well with others and is good at staying on the assigned tasks. She will need direct supervision and verbal prompting at times.
