Education Specialist Credential Program (Mild/Moderate)

STUDENT TEACHING APPLICATION

DEADLINES: Applications are due to the Student Teaching and Field Placement Coordinator no later than October 1st for Spring Student Teaching and March 1st for Fall Student Teaching. You must schedule a phone or in person appointment with the Student Teaching and Field Placement Coordinator to review your application. Late applications, within 1 week of the deadline, will be accepted with an additional $30.00 late fee. During your application appointment you will sign up for your Student Teaching Interview.

STUDENT TEACHING APPLICATION

1.  Student Teaching Applications must be typed and complete.

2.  In accordance with state polices, the School of Education at Biola University is building collaborative partnerships with school districts. Consequently, placements will be made on a rotational basis in the districts where we have developed these collaborative partnerships in order to assure our students the highest quality placements during their student teaching experience.

3.  Special Education candidates should complete a placement request for the area in which you desire to complete your student teaching. The Field Placement Coordinator will then secure a placement for you based on your request.

4.  Contracts contained in this packet must be signed and dated.

5.  In order to withdraw your application, you must contact the Student Teaching and Field Placement Coordinator immediately, submit a formal withdrawal letter, and submit required withdrawal fees outlined in the contract.

6.  After approval for student teaching, remember to register for 12 units of student teaching. Information on student teaching courses will be sent to accepted students, via email.

PERSONAL DATA FORM

This form must be typed. After approval, it is sent to the placement school; it should be carefully composed and free of spelling or sentence structure errors. Please submit the Personal Data Form with a photograph with your application. Photos should be a passport-type photo (i.e. head and shoulders only).

My.Biola ACADEMIC TRANSCRIPT

Attach a CURRENT unofficial transcript. The transcript must include your current cumulative GPA and current courses being completed. Please note: a minimum overall GPA of 3.00 is required to be eligible for student teaching.

COPIES OF PROFESSIONAL DISPOSITION AND COMPETENCIES EVALUATIONS

Students with low professional disposition ratings who are not accepted into Student Teaching will be required to demonstrate proficiency on all competencies by: retaking the course OR enrolling in a one unit 60 hour fieldwork course. This decision will be made at the discretion of the Director of Special Education.

TEST RESULTS

You must submit applicable score reports. All tests must be completed and passed prior to applying to student teach. Submit one of the following: Office CBEST and CSET scores, SAT, ACT, AP Exam scores. (See SOE for minimum score requirements for SAT, ACT, and AP Exams).

CURRENT NEGATIVE TB TEST RESULTS

Due district policy, you must submit proof of a negative TB test result within 6 months of submitting your application.

U.S. CONSTITUTION

A grade of “C” or higher must be achieved to meet the U.S. Constitution requirement. If you met this requirement though a course at another college/university, you must submit proof the course covered the U.S. Constitution. This can be accomplished by submitting a course description, syllabus, or letter on school letterhead that shows the course covered the U.S. Constitution. The School of Education offers exams every February and September if you have not yet met this requirement.

CERTIFICATE OF CLEARANCE

A Certificate of Clearance should already be on file in the School of Education Office. If this has not already been submitted, contact a Credential Analyst in the School of Education for immediate assistance. Completion of the Certificate of Clearance process is required in order to be approved for Student Teaching.

I hereby apply for admission to student teaching. If my application is accepted, I agree to abide by the following requirements and policies. My initials indicate I have read, understand, and accept the following:

In accordance with California credential requirements, I understand one placement will be in a primary setting and one in a secondary setting. If I do not satisfactorily complete all student teaching requirements with a grade of “B-” or better, an additional placement may be required.

Student Teaching at a private school must take place at WASC-accredited school and requires submission of a full-time employment contract and an additional summer placement at a public school.
______/ 5.  In addition to the traditional school day, I will arrive at my placements at least thirty minutes prior to the beginning of class and spend at least thirty minutes per day at the school conferring with my supervising teacher, preparing materials, and carrying out other indicated teaching functions. I understand that I am expected to attend Back to School Night (fall) or Open House (spring).
