CAPSTONE PROJECT REQUEST FORM for Graduate Students

Dear Student and Faculty Sponsor:

The second page of this document is a capstone project request form for graduate students. Use this form prior to or during a semester/session when registration for an individualized degree capstone thesis or special project course is planned. Submission of this material notifies the University’s scheduler that a unique course needs to be created for a student during a particular period of time because it is not available as a standard offering. The form is being provided to you as an emailed item for your convenience and to save mailing costs. The graduate student and sponsoring faculty member may complete the capstone project request form for administrative approval in either of two ways. The document may be printed, filled out using a typewriter, signed by the parties, and sent forward to the department chair.

The student and faculty sponsor, together, may also complete the capstone project request form electronically. The student fills in biographical, program, and other information through to the “student status” (full-time, part-time) section. The form may then be saved and emailed to a sponsoring faculty member. The faculty member, or faculty member and student, then work(s) to complete the project details sections. The parties may dialog about these items until a project is briefly described to their satisfaction in the space provided.

When completing a capstone project request form electronically, use a computer to type in or check off the form’s fields. Answer boxes may be noted by clicking the mouse. Shaded areas are text fields for typing. Jump from field to field by using the tab key or positioning the mouse. If the student and sponsoring faculty member choose to complete the capstone project request form using their computers and email, a preliminary packet may be saved for individual records before signatures are added. This file may be emailed as a message attachment. Hopefully, students and faculty members will have each other’s email addresses on hand. When the request is agreed upon, student and faculty sponsor sign off and send it forward to any other University officials who also need to review and concur with the capstone proposal.

The Graduate Studies Office will provide fully approved copies as specified and the Enrollment Center will contact the graduate student through the regular mail system with any additional details related to course fees and scheduling.
