VOLUNTEER SERVICE AWARD

FOR OUTSTANDING SERVICE IN THE FIELD OF HUMAN SERVICES

Instructions

The Concord-Carlisle Community Chest Annual Service Award is given to students who have demonstrated initiative and commitment to a volunteer project within the area of human services. A minimum of 10 volunteer hours performed between June 1, 2014 and April 1, 2015 is required for consideration. The recipients are selected by the Community Chest to recognize and perpetuate service and which embodies our mission of neighbors helping neighbors. This award is available to all high school students.

ELIGIBILITY

The nominee must be either a Concord or Carlisle resident, or attend public or private high school in Concord.

PROCEDURE

Nominations are solicited from students, faculty, or anyone in the community who works with teens. Students are encouraged to submit an application for themselves as candidates. Applications should be submitted to the Community Chest by April 10, 2015 either by email to or by mail to:

EXAMPLES

· Student provides exceptional service in a hospital or nursing home.

· Student participates in frequent preparation and serving of meals in a soup kitchen.

· Student works one-on-one with a special needs child.

CRITERIA

1. Candidate must be a high school student in good standing. (There are no additional academic requirements).

2. The candidate must demonstrate his/her initiative and commitment to the project. A minimum of 10 volunteer hours towards a project performed between June 1, 2014 and April 1, 2015 are required for consideration.

3. The project must be voluntary and in the human services area, serving an individual, family, or on-site at a human services agency.

AWARD

A framed citation and letter of commendation will be presented to the Honoree at her or his school’s Awards Night.
