POSITION TITLE: Nurse Care Manager

GENERAL DESCRIPTION OF DUTIES: The Nurse Care Manager will work in the behavioral health setting to provide routine nursing care services, screening and assessment, client and staff education, and will support clients in connecting to other services as necessary to meet their medical needs. The Nurse Care Manager may be assigned to a specific program as needed; in all instances, the Nurse Care Manager will be part of a multi-disciplinary team.

SUPERVISION RECEIVED: Receives clinical and administrative supervision from the program Manager or designee.

COLLABORATES WITH: ACFS providers, community providers, local and state agencies, and other staff within the Agency.

PERFORMANCE EXPECTATIONS:

1. Develops and implements plans with clients and providers as appropriate.

2. Provide routine nursing assessments that may include but are not limited to: vital signs, physical assessment, assessment of medication side effects and monitoring lab values.

3. Provides clear and consistent verbal and written communication with prescribers, medical offices, and other providers both internally and externally to the Agency.

4. Adheres to CDOI (Client Directed Outcome Informed) practice in working with clients and

participates in CDOI training as appropriate.

5. Maintains client record documentation for delivered services according to Agency policy.

6. Provides verbal and written educational instruction to clients and staff on health management topics, medication side effects, and chronic diseases.

7. Administers and monitors prescribed medications to clients as needed and as appropriate within nursing scope of practice.

8. Supports clients through written and verbal communication both in person and on the telephone.

9. Meets with clients at the office, in their homes, and in the community as assigned, including accompanying clients to medical appointments as needed and as appropriate.

10. Participates in team meetings as needed and as appropriate.

11. Represents the Agency in community meetings as assigned

12. Functions in a cost-effective manner with regard to time utilization, mileage and job related expenses.

13. Performs other related duties as assigned.

14. Complies with Agency and mandated safety procedures and regulations for the good of employees and clients.

ESSENTIAL KNOWLEDGE, ABILITIES AND SKILLS:

QUALIFICATIONS:

Education/Experience/Licensure/Certification: This position requires a Bachelor’s degree in nursing and a Registered Nurse licensure. A Certified Care Manager certificate is preferred. In some programs, ANCC certification in psychiatric mental health (RN-BC) may also be required. In addition, the position requires two years of experience in medical case management; previous experience in community-based mental health services is preferred. Must possess a valid Maine driver’s license and a reliable means of transportation with a minimum of $100,000/$300,000 auto liability insurance.

SKILLS AND KNOWLEDGE REQUIRED:

· Ability to perform routine nursing care services, e.g., vital signs, and assessments within the scope of Maine State Nurse Practice Act.

· Ability to participate in coordinating care across multiple medical providers on behalf of the client.

· Ability to do outreach to external primary care providers and develop professional relationships within the community.

· Ability to participate in coordination of referrals to enabling services, wellness programs, chronic disease management and monitors follow through.

· Ability to provide training and education to Agency staff as well as clients regarding medical issues impacting the client population.

· Ability to work with clients and their natural supports to develop care plans.

· Ability to accurately collect record and monitor patient outcome data.

· Ability to provide therapeutic treatments and procedures according to established care plans and department protocols.

· Must have an understanding of chronic conditions and care management skills, which support positive client outcomes.

· Ability to function as a member of the multi-disciplinary team, encouraging and maintaining professional communication and continuity of care.

· Computer literate and proficient in keyboarding skills for data entry into electronic medical record.

Physical and Environmental Conditions and Requirements:

The conditions and requirements described here are representative of those that must be met by an employee to successfully perform the essential functions of the job.

Workplace Environment:

The employee may occasionally be exposed to environmental pollutants (e.g., pet dander, smoke), outdoor weather, moderate to loud noise, crowded office conditions with little privacy. Certain work assignments may include the need to work in clients’ homes and out in the community. The employee will occasionally be required to work overtime, evenings, weekends, or unpredictable schedules. Some programs may require participation in an on-call rotation. The employee must be able to work regular full-time (40 hrs per week) as scheduled.

Physical Requirements: Must have either corrected or uncorrected binocular vision of at least 20/40, a hearing threshold of less than or equal to 35 dBa at 2000 HZ in both ears, and must be able to stand for 30 minutes. The employee must be physically able to stand; walk; sit; use hands to finger, handle or feel; climb stairs or balance; stoop, kneel or crouch; crawl; pull and push; carry and grasp; reach up to shoulder high; bend and twist at the waist; walk on uneven ground; lift up to 25 pounds; write/document outcomes of meetings in a

timely manner; and type and input data into a computer more than 4 hours per day. Occasionally, the employee will be required to sit and ride in a vehicle; transport self to another office or service area at some distance with individual trips of up to 2 hours and an average of 200 or more miles of driving per month; and drive in varying weather conditions.

Communication Requirements: The employee must be able to talk on the phone and in person in conversations that may involve individuals or small groups; hear, write, and read. Vision requirements include close, distance, and peripheral vision, depth perception, and ability to adjust focus.

Mental Requirements: The employee must be able to get along with others; meet deadlines and productivity standards, complete paperwork on time; respond to varying situations with tact and diplomacy and deal with stressful, hostile, or irrational persons, whether due to physical or mental disability, drugs, socio-economic differences, or other factors; react calmly and effectively in emergency situations; and remain flexible in changing circumstances and situations.

JOB PERFORMANCE EVALUATION:

Employee must satisfactorily complete a 6-month probationary period. At that time, employees may be granted regular status. Subsequent job performance evaluations will occur on an annual basis, on or before the anniversary of the employee’s original date of hire.
