Dear Parents,

Your child may have been exposed to one of the following communicable diseases:

___ Conjunctivitis (or pink eye) –common symptoms include redness and swelling of the membranes of eyes with burning or itching, thick yellow discharge and sensitivity to light.

___ Whooping Cough – common symptoms include a cold with runny nose, sneezing, low grade fever and cough which last for several days.

___ Chicken Pox – common symptoms include skin rash, which progresses to blisters, then scabs.

___ Strep Throat – common symptoms include fever, sore throat, and oozing and redness of tonsils and throat.

___ Shingles – common symptoms include rash-reddened, raised spots that turn to blisters and later form a scab, general discomfort and fever.

___ Scarlet Fever – common symptoms include fever, nausea and vomiting, red throat with pus spots, tender and swollen lymph nodes, rash on skin and inside of mouth.

___Hand, Foot, and Mouth – common symptoms include raised rash on palms, soles of feet and surrounding mouth, which progress to blisters, scabs, sores inside of mouth and sore throat.

___ Head Lice – common symptoms include itching of head with associated scratching, scratch marks on the back of the head and behind ears. Common locations for head lice are back of neck and behind ears. Nits are usually found close to the root of hair strand.

If your child has any of these symptoms, please keep your child home and contact your physician. If you child has a confirmed case, please contact your child’s teacher.
