CERTIFIED LAY MINISTER
CERTIFICATION PROCESS CHECKLIST
CAL-PAC ANNUAL CONFERENCE
____ Meet with home church pastor to discuss call and discern whether your call might lead
(Date) to becoming a Certified Lay Minister in the United Methodist Church.
____ Obtain CLM Form 03: “Certified Lay Ministry Recommendation Form” and become familiar with
(Date) the process and forms for entering the process of obtaining Certified Lay Minister certification.
____ Pastor completed and signed CLM Form 03
____ CLM Form 03 delivered to Church (Administrative) Council/ Charge Conference
____ Apply in writing to the Church (Administrative) Council or Charge Conference from the home
(Date) church where you hold membership and be recommended by vote of the Church
(Administrative) Council or Charge Conference.
____ Secretary, Church (Administrative) Council, completed and signed CLM Form 03
____ CLM Form 03 delivered to District Superintendent (District Office)
____ Complete all approved education requirements, i.e. Lay Ministry Academy.

____ Obtain a current standard background check – a release form needs to be signed – through the
(Date) Annual Conference Human Resources office. Attach background check results to application
packet submitted to District Committee on Ordained Ministry. (One will be sent to them as well.)
____ Meet with District Superintendent to discuss call and receive recommendation to the District
(Date) Committee on Ordained Ministry.
____ District Superintendent completed and signed CLM Form 03
____ CLM Form 03 delivered to District Committee on Ordained Ministry
____ District Superintendent enters the candidate for psych testing (see CLM Form 04)
____ Go through psychological testing, which includes the MMPI and MCMI3 administered by the
(Date) Conference Ministerial Assessment Specialist (see CLM Form 04). You must obtain a proctor for
the testing (contact the District Office). The proctor is responsible to obtain test and set up the testing system. The California Ministerial Assessment Specialist will contact candidate to set up an appointment for the oral interview. Results go to the candidate and to the District Committee on Ordained Ministry chair. This process changes periodically; research the process (GBHEM).
____ Apply in writing to the District Committee on Ordained Ministry (DCoM). Application
(Date) packet should include; a three-page statement of how the Lay Academy and concurrent
ministry involvement has shaped and confirmed your calling to Certified Lay Ministry
(this could include a cover letter, one page describing your call, and one page
describing your educational preparation) and three letters of reference. Letters of
reference should be from those who know your work and personal character, including
your best skills and examples of where they have observed you in service.
____ Appear before the District Committee on Ordained Ministry for review and approval.

ONCE CERTIFIED, THE CERTIFIED LAY MINISTER MUST:
(See CLM Form 07 “Certified Lay Minister Annual Report” for recording reviews, recommendations and continuing education dates and information for continuing certification.)
____ Annual ministry review by the Church Council or Charge Conference where the CLM is assigned.
Annual Report CLM Form 07to be sent to District Superintendent, District Committee on
Ordained Ministry, and Director of Leadership.
____ Annual recommendation of the District Superintendent.
____ Every other year: satisfactory completion of an approved Continuing Education event (ten hours
minimum) and report sent on CLM Form 07, to District Superintendent, District Committee on
Ordained Ministry, and Director of Leadership.
____ Every other year: review by the District Committee on Ordained Ministry or their appointed
ad-hoc committee. The District Committee on Ordained Ministry may ask to meet with you
annually.
IF TRANSFERRING FROM ANOTHER CONFERENCE
____ Meet with Lay Ministry Academy Steering Committee representatives for approval of education requirements.
