I. Introductions and Roll Call / Roll call was taken and the meeting was called to order. The minutes from the July collaborative call were reviewed. There were a couple of changes to the July minutes. / Update and finalize the July meeting minutes / HSAG / 10/26/11 / 10/26/11
II. Activities IX and X / HSAG reviewed the documentation requirements and the scoring of Activities IX and X.
· The plans should fill out the results table in Activity IX of the PIP Summary Form. The numerators, denominators, and statistical testing results for every study indicator should be provided.
· For Activity IX, HSAG looks for improvement between each measurement period and statistically significant improvement between each measurement period for every study indicator. HSAG also assesses whether the same methodology was used for baseline and remeasurement. If there was a change in the methodology, the plan must provide justification.
· For Activity X, HSAG assesses for sustained improvement from baseline. The PIP should have demonstrated improvement and sustained it for another measurement period. No additional documentation is required in Activity X. / NA / NA / NA / NA
III. Review of the Collaborative Intervention / The PMHPs, including Magellan Health Services, provided updates on getting in contact with members.
FHP reported an increase in the number of members they are unable to reach; the phone is disconnected, they can’t leave a message, or someone else answers the telephone. This is their biggest barrier. However, FHP has not seen an increase in the number of members who have refused further treatment.
Some of the plans reported having more contact with members. For example, Magellan is doing the “welcome call” which is a case manager calling each member upon discharge to follow-up on their needs so they can follow up with their care. Magellan also is improving on Bridge Appointment follow-up.
University of Miami reported 99% compliance with Bridge appointments.
Some of the barriers encountered are that plans/agencies may not have updated information on members. Magellan is currently updating their membership contact information.
ABH identified a problem that they were able to correct with gap analysis. One of their major facilities was not always communicating information about the 7-day follow-up appointment. Members were being told about the group that was held every Wed., rather than a specific date. With a correction of this issue, more members are now showing up for their 7- day follow-up appointment. Even members with no phone number are showing up. / NA / NA / NA / NA
IV. Subgroup Analysis / HSAG reviewed the subgroup analysis worksheet that was sent to the PMHPs prior to the call. HSAG explained that each PMHP will need to complete a subgroup analysis of their collaborative PIP rates and present the results at the January 11, 2012 onsite meeting. / Complete subgroup analysis / PMHPs / 12/15/11
