Only SNA certified members should sign this roster. Should be used by educational provider and returned to SNA, within 30 days of course, in order for participants to receive CEUs. Pre-approval for all continuing education units must be requested. Identification number is required to receive CEUs, if membership number is not listed the participant will NOT receive the CEUs. Copy as needed.
