ABBREVIATED GENERAL ADULT SERVICES CQA TIP SHEET

· The SSW may utilize this format for General Adult Service Cases when the Cabinet receives a request from the court to conduct an Interdisciplinary Evaluation (IDT) and the adult identified in the request is alleged to be partially disabled or disabled as referenced in KRS 387.540 and abuse, neglect or exploitation are not alleged. This tipsheet is not to be utilized for reports that are accepted for General Adult Service Assessments.

· The FSOS has the option of excluding any referral from this abbreviated format at any time.

When the Abbreviated General Adult Services CQA is used, the information listed below must be included in the following sections:

· Maltreatment/Presenting Problems Screen: Include the following statement “An Interdisciplinary Evaluation (IDT) report has been accepted for services and abuse, neglect, or exploitation is not alleged. This report is being documented using the abbreviated General Adult Services CQA format. See Referral Results Summary Screen for assessment findings.” By including this, anyone reading this CQA will understand why the format being used is different from the typical CQA.

· Family Functioning: NA See Referral Results Summary Screen for assessment findings.

· Screening: NA See Referral Results Summary Screen for assessment findings.

· Support: NA See Referral Results Summary Screen for assessment findings.

· Sequence: NA See Referral Results Summary Screen for assessment findings.

· Referral Results Summary Screen:

Include the following: “An Interdisciplinary Evaluation (IDT) report has been accepted for services and allegations of abuse, neglect or exploitation are not alleged.” This report is being documented using the abbreviated General Adult Services CQA format. Staff is instructed to insert a copy of the IDT assessment in this section.
