o We will be requiring CoCs to report on bed coverage and data quality. ECHO HMIS staff will be monitoring
o Shelter systems will be affected in regards to data collection policy

o Community Care specifically tied to Travis County (24-26 clinics in the county)
o CC is a Public Entity Model Federally Qualified Health Care (FQHC)and works closely with Central Health
o Targets “Safety Net” population (primary care, internal medicine, family practice, pediatrics, behavioral health-with ATCIC; dental care
o Current partnership with ATCIC-Section 11.15 (SS Act) Waiver granted to state of Texas and receiving the waiver from the federal government to match every dollar of governmental funds with a $1.60-if successful outcomes are demonstrated
o ATCIC provides “telepsychiatry” in their psychiatrist homes
o Dental is only provided for medical clients
o Mobile Health Teams-looking for places to go (shelter and PSH)-team can do screenings, primary care, and bringing patients into the system who have not have primary access

o Looking to grow in NE Austin and expanding location, Comina LaCosta (between ITT Tech and SSA office)
o Level III certification clinic-Patient Centered Medical Certification
o FQHC-relies heavily on Medicaid population
o Integrated Behavioral Health Program

o Looking for NE quadrant locations
· All people experiencing homelessness can access resources at any Community Care site
· ATCIC: One on One relationship between providers; formalize clear and direct relationship
· Q: Is there a formalized document that the CoC that will cover core elements ? A: Resist urge to be very detailed, and focus on broad initiatives
· Local HUD applications will have question re: MOU with healthcare, by next year, all CoC’s must have formal MOU’s with primary healthcare provider and include as formal ranking criteria
