Summer CampEnrollment Form
The cost of Summer Camp is $125.00 weekly per student or $30.00 daily per student.To hold your child’s reservation; Tuitionof $125.00 or for specified days, $30.00 per day, is due on the Thursday prior to the week of participation. If tuition is not paid by Thursday (prior to week of attendance) charges will be assessedat $35.00 dollars per day or $150.00 for the week and due upon arrival.
The Summer Campis structured for students whom are 5 years old as of June 6th(Enrollment year) and up to 8th grade. Our program hours are 7:30am-5:30pm. If your child needs earlier drop off, this will be available at preschool at an extra charge of $8.00. Children arerequired to bring a snack and sack l daily. All accounts must be satisfied and paid in full to be able to attend summer camp.
This agreement also allows my student/students to participate in Summer Campas posted. If any student should not be allowed to participate in the daily activity either by parentalrequestin writingor by the administrator’s discretion, the parent will be required to withhold that student from attending camp for that day. Any items brought into the facility shall not be representative of non-Christian like matter such as; games, music, periodicals or magazines, toys, clothing, movies, or any other venues. Any items in question will be held in the office and returned home that day. *We are not responsible for any lost or stolen items. If it is valuable, please leave it at home.
For your convenience we accept Visa, Master Card, Checks or Cash. At the time of enrollment, credit card is required. By signing this Agreement, you are giving D.J.’s CommunityChristianAcademy permission to use credit card for non-payment of all monies owed.
“Tuition will not be refunded if, for any reason, you decide to withdraw your child”.
There are no refunds for days missed due to illness, Holidays, or restrictions. This space has been reserved for your child and staff has been hired accordingly. Days missed due to illness, Holidays, or restrictions may not be traded for future dates.
