SMALL OPERATOR ASSISTANCE WORK ORDER

1. All test holes for this SOAP Project will be logged and bagged by ,

No payment for a particular item will be made unless the particular item is designated in this work order. The agreed project ceiling of this project is . PROJECTS NOT DRILLED WITHIN 45 DAYS ARE AUTOMATICALLY TERMINATED UNLESS AN EXTENSION OF TIME IS REQUESTED AND APPROVED IN WRITING. EXTENSION OF DRILLING TIME WILL BE APPROVED ONLY DUE TO ADVERSE WEATHER CONDITIONS.

I understand that failure to provide the required drilling services within 45 days will result in the termination of this work order and the assessment of liability for all costs incurred by the Department as a result of services rendered by the consultant. I agree that reimbursement liability shall apply to the costs of all work performed in accordance with work orders and work order amendments required to develop a complete project report.

Prior to the initiation of any drilling in association with this work order, a properly completed 5600-FM-BMP0092: Notice of Intent to Explore or Request For Permit Waiver, must be submitted to the appropriate District Office.
