4-H ROOKIE OF THE YEAR
State Award
Award: Plaque (8x10)
The Rookie of the Year Award may be presented to one individual whose appointment is at least 50% 4-H. They must have demonstrated outstanding performance as a professional and provided outstanding support to the development of the county 4-H program.
REGULATIONS:
qEligibility is open to any current member of the Florida Association of Extension 4-H Agents who has served as a 4-H Extension Agent or Courtesy Agent for more than two (2) years, but less than four (4) years.
q50% of programming efforts must be in 4-H.
qApplications must be received by the Special Awards Chairperson by March 1 of current year.
qNominations may be from administration, County Directors, FAE4-HA District Directors, Specialists, or other agents.
qIndividuals may not nominate themselves.
qThe FAE4-HA Special Awards Committee will serve as judges. If one of them is nominated, they will not judge the category.
qJudges will consider 4-H program support and professional performance in selecting award recipient.
qJudge’s decision will be final.
qThe award will be presented at the Annual FAEP Meeting.
SUBMISSION REQUIREMENTS:
qTop of page should include title of award, nominee’s name and county
qApplication should be limited to two typed pages describing the nominee’s program accomplishments. This can be through personal collaboration or from applicant’s ROA.
qApplication is to be signed by the person making the nomination, including title, address, phone number, and e-mail address.
qInclude one 2" x 3" photo along with application.
q Mail original application and three (3) copies to Special Awards Chair by March 1.
FAE4-HA LITERATURE AWARD
State Award
Award: Plaque (5x7)
The FAE4-HA Literature Award will be presented to the author(s) of materials (new or with major revisions) utilized to support successful 4-H programming at the county, district, or state level. All materials (handbooks, project manuals, project records, leader’s guides, etc.) submitted must have already been implemented in support of a successful 4-H program.
REGULATIONS:
qLiterature Award Applications must be submitted to Special Awards Chair within one year of
its printing or major revision date.
qApplications must be submitted to the Special Awards Chair by March 1.
qMay be an individual or team effort, with at least one person of the team a current member of
FAE4-HA.
qEligibility is open only to county extension agents (Courtesy and State), not specialists.
qNo more than five (5) FAE4-HA Literature Awards may be presented each year. The judges will determine how many will be awarded, and reserve the right to make the recommendation that none of the entries receive an award.
qOne plaque will be award to team entries along with a framed certificate for each team member.
qTeam entry must show evidence of joint planning and implementation of the materials.
qProgram must have been conducted within the past 12 months.
qThe judges’ decision is final.
SUBMISSION REQUIREMENTS:
q Application must be limited to two typed written pages illustrating the following:
*documentation stating how program meets the need of the community
*objectives of the program
*methodology stating how program was implemented
*how was program evaluated and discuss the impact
*show how program is/can be an on-going program
*show any collaboration with other agencies (if applicable)
q Include in cover page title of literature, name of individual/team members, addresses, county, phone and e-mail address.
q Submit original and 3 copies to Special Awards Chair by March 1
FAE4-HA SEARCH FOR PROGRAM EXCELLENCE AWARD
State Award
Award: Plaque (5x7)
The FAE4-HA Search for Program Excellence Award will be presented to the Extension 4-H Agent with outstanding county program accomplishments as evidenced through success stories or narrative progress reports.
REGULATIONS:
qNo more than five (5) awards will be presented each year
qApplications must be submitted to the Special Awards Chair by March 1.
qApplications must be submitted to the Special Awards Chair within one year of the date submitted to the state as a success story or narrative report within the annual Report of Accomplishment.
qEligibility is open to only current members of FAE4-HA.
qMay be an individual or team effort, with at least one person of the team a current member of
FAE4-HA
qOnly one entry per person/team may be submitted for judging.
qJudges decision will be final.
qAwards will be presented at the Annual FAEP Meeting
qTeam entries must show evidence of joint planning and implementation of two or more individuals.
qOne plaque will be awarded to team entries, with each team member receiving a framed certificate.

FAE4-HA OUTSTANDING SPECIALIST AWARD
State Award
Award: Plaque (8x10)
The FAE4-HA Outstanding Specialist Award will be presented to an individual who has provided outstanding support to Extension 4-H Agents, or major phase of the 4-H program.
REGULATIONS:
qEligibility is open to any Florida Extension Specialist who provided support to 4-H.
qApplications must be submitted to Special Awards Chair by March 1.
qNominations will only be accepted from FAE4-HA members (county faculty) only.
qOnly one award will be awarded each year.
qPrevious award recipients will be eligible for this award again provided their nominations are not a duplication of previously recognized accomplishments .
qThe judges decision will be final.
qAward will be presented at the Annual FAEP Meeting

*how nominee has supported the 4-H program.
*identify areas of support of youth development
*program and career accomplishments
qMust include one 2x3 photo with application.

qTitle page should include name of nominee and title, name of award, name of person nominating, job title, county, address, phone and e-mail address.
FAE4-HA FRIEND OF 4-H
State Award
Award: Plaque (8x10)
The FAE4-HA Friend of 4-H Award is given to any volunteer, business, or community leader (non-extension employee) who has significantly benefitted 4-H programs in more than one (1) county area and shown a commitment to 4-H for more than two (2) years in eligible to be nominated.
REGULATIONS:
qApplications are due to the Special Awards Chair by March 1.
qNominations will only be accepted from FAE4-HA members.
qOnly one award will be presented each year.
qThe judge’s decision is final.
SUBMISSION REQUIREMENTS:
qApplication must be limited to no more than two (2) typed pages describing the following
* show evidence of support of the 4-H program
* personal background and length of support of 4-H youth development
qMust include one 2x3 photo with application
qSubmit original application and three (3) copies to the Special Awards Chair by March 1.
qTitle page must include name of award, name of nominee and county, name of person submitting application, job title, address, phone and e-mail address.
