Re:PHCS Provider Network Replaces CCN Network
Dear PPO Plan Member,
To ensure that all of our members have the greatest access to care outside of the Plan’s service area, the Plan contracts with a national network of healthcare providers. This network is chosen with great care by researching national provider networks offering comprehensive provider networks, quality credentialing standards and exemplary customer service. Beginning January 1, 2006, the out-of-area provider network will change from the CCN Network to the PHCS Network.
Since your employer offers an Optima Health plan as an insurance benefit to its employees, it will be necessary for you to use the PHCS provider network for your out-of-area healthcare needs for services to be paid at the in-network level of benefits. You are still free to use any provider on an out-of-network basis.
The PHCS Network is the largest proprietary provider network in the country with nearly 450,000 providers and 4,200 facilities nationwide. PHCS has negotiated reimbursement arrangements with the providers in its Network that result in significant cost savings and when you visit an in-network provider these savings are passed on to you, helping you to maximize your level of benefits.
With a network this size, we are confident that if your current physician is not a PHCS provider, you will easily find a network physician partner who will meet your needs. To find out more about PHCS or to locate a PHCS provider, visit the PHCS website at or by contacting them at 1-888-817-7427 Monday through Friday from 8 a.m. to 8 p.m. When asked to select a network, choose Preferred Provider Organization (PPO).
If you wish to remain with your current physician and find that he or she is not participating with PHCS, you can request that he or she be added to the network by submitting a provider nomination form. Visit the PHCS website for a copy of this form.
You will be receiving a new Member ID card that will include the PHCS information. This new card will enable PHCS providers to easily identify you as a Plan member.
