Service Agreement

Convenience Driving School, hereinafter referred to as “Driving School” agrees to provide applicant, hereinafter referred to as “Student”,

*All prices are cash only, a 3.5% processing fee be added to cost of the package, course, class or any program we offer when paid with credit card, or debit card. The price in brackets is the cost of the package, course, class or any program we offer plus the processing fee. Price listed in ($) has the processing fee added.

*Fuel Surplus Fee will be added in addition to the cost on any course, program, course, class or any program we offer in which the driver education car is used. The fee is $2.00 per lesson or per hour. The fuel surplus fee is price underlined at each package which the surplus fee is added.

Classroom Only Package, $275 ($284.63) - 36-Fifty (50) minute in-class Driver Education lessons, CLASSROOM ONLY INSTRUCTION which meets or exceeds standards set by the Virginia Department of Education (DOE) and Virginia Department of Motor Vehicles (DMV).*

Under 19 years of age complete Driver Education Package, $450 ($465.75) - 36-Fifty (50) minute lessons in-class instruction, seven (7)-fifty (50) minute observation and seven (7)-fifty (50) minute in-car behind-the-wheel driving instruction on the seventh lesson student will take DMV road test. If student passes the road test the student qualifies to receive their driver license once all DMV requirements are completed. All instruction meets or exceeds standards set by the Virginia Department of Motor Vehicles (DMV). REQUIRED FOR ALL STUDENTS UNDER 19 YEARS OLD.*

In-Car, On-Road under 19 years old only Package $250 ($258.75) – 7-fifty (50) minute in-car behind-the-wheel lessons and seven (7) fifty (50) minute in-car observing another Student driving which meets or exceeds standards set by the Virginia Department of Motor Vehicles (DMV). Once Student completes the on-road portion and passes DMV road test Student will receive certificate of completion.*

Three Times Failed under 19 years of age Knowledge Test Package $250 ($258.75) - 36-Fifty (50) minute period, CLASSROOM ONLY INSTRUCTION which meets or exceeds standards set by the Virginia Department of Education (DOE) and Virginia Department of Motor Vehicles (DMV). Once Student completes the course, the Student will receive a certificate which will be presented to DMV to re-take knowledge test.*

Three Times Failed under 19 years of age Road Test Package $275 ($284.63) - 7-fifty (50) minute sessions in-car behind-the-wheel driving instruction with licensed instructor in which meets or exceeds standards set by the Virginia Department of Motor Vehicles (DMV). Student must hold valid learner permit to complete in-car behind-the-wheel. On the seventh lesson the student will receive the road test, should student pass road test student receives their driver license.*

Three Time Failed 19 years of age or over knowledge test Package $175 ($181.13) - 8-eight hour classroom course based on the Virginia Driver's Manual. The course meets or exceeds the standards set by DRIVE TO WORK. Once student completes the course the student will receive a certificate to re-take the road exam again.*

Three Time Failed 19 years of age or over Road Test Package $275 ($284.63) -7-fifty (50) minute sessions behind-the-wheel driving instruction with licensed instructor which meets or exceeds standards set by the Virginia Department of Motor Vehicles (DMV). Once student completes the course the student will receive a certificate to present to DMV to re-take the road exam.*

Prepare Student for DMV knowledge, sign, and road test package $250 ($258.75) – (4) four hours of in-class instruction on Virginia Driver Manual to prepare student for DMV knowledge test. (4) four hours of in-car instruction to prepare student for DMV road test. Student must have valid learner permit to receive the in-car, behind-the-wheel instruction. If student, and instructor agree Student requires more instruction to pass the knowledge test the student can continue in-class instruction for $30/hour, if Student and instructor agree student requires more in-car instruction for $40/hour.*

Prepare Student for DMV knowledge test $250 ($258.75) – (8) eight hours of in-class instruction on the Virginia Driver Manual to prepare student for the knowledge and sign test. Class. If student, and instructor agree student requires more instruction to pass the knowledge test the student can continue in-class instruction for $30/hour. Student does not receive three times failed certificate.

Prepare 19 years of age or older Student to pass driver test $375 ($388.13) - Student with valid learner permit will be given 12-50 (fifty) minute driving lessons in-car behind-the-wheel instructions. If Student and instructor agree student requires more instruction to pass the test the student can continue in-car instruction for $40/hour.*

Prepare 19 years of age or older Student to pass driver test $200 ($207.00) - Student with valid learner permit will be given 6-(50) fifty minute driving lessons in-car behind-the-wheel instructions. If Student and instructor agree student requires more instruction to pass the test the student can continue in-car instruction for $40/hour.*

19 years of age or older Student receive driver license, complete program (DTSD Form) $500 ($517.50) – Students who are 19 years or older with valid learner permit, to receive driver license, waive 60 day hold for license, and will not be required to take road test at DMV. Complete 36-(50) fifty minute In-Class Driver Education lessons, completes 7-(50) fifty minutes lesson in-car behind-the-wheel 7-(50) fifty minute lessons observing another Student drive, and pass road test will receive the DSTD form. Student takes the DSTD form to the DMV customer service center to receive their driver license.*

19 years of age or older Student receive driver license (DTSD Form) $325 ($336.38) – Students who are 19 years or older with valid learner permit has proof of completing In-Class Driver Education (DEC-1), or has completed In-class Driver Education with Driving School. Student will complete 7-(50) fifty minutes lesson in-car behind-the-wheel 7-(50) fifty minute lessons observing another Student drive, and pass road test will receive the DSTD form. Student takes the DSTD form to the DMV customer service center to receive their driver license. Student which completes the requirements to receive a DTSD is not required to hold learner permit for 60 days. *

19 years of age or older Student receive driver license, form only (DTSD Form) $25 ($25) – Students who are 19 years or older with valid learner permit has proof of completing In-Class Driver Education (DEC-1), or has completed In-Class Driver Education with Driving School. Student has proof of completing 7-(50) fifty minutes lesson in-car behind-the-wheel 7-(50) fifty minute lessons observing another Student drive, or has completed In-car behind-the-wheel with Driving School, and passed road test will receive the DSTD form. Student takes the DSTD form to the DMV customer service center to receive their driver license. Student which completes the requirements to receive a DTSD is not required to hold learner permit for 60 days. *

Special class, course, or package offered to meet Students schedule (Does not apply to 36 lesson course) $100 ($103.50)-For Students which need or want to complete a class, course, program or package at a special scheduled time. Driving School will schedule the class, course, program, or package to meet the Students schedule for additional fee. The fee is in addition to the cost for the class, course, or program. DOES NOT APPLY TO 36 LESSONS.

Special class for 36 Lesson Driver Education Course $200 ($207)-If the Student request to complete the 36 lesson course before the next scheduled class. Driving School will schedule the class at the next available time so Student can complete the course as soon as possible. This fee is in addition to the cost to the cost of the package.*

Use driver education car to take road test at DMV $150 ($155.25)-Student will be taken to the DMV to take their road test. Student will use Driving School car to take DMV road test, then Student will be brought back to agreed location.

Fuel Surplus Fee ($2.00 x ______= $______Upon the date of agreement, if the price of gasoline is $3.50 or greater at the gas station which Driving School uses , a fuel surplus fee will be added in addition to the cost of the package, course, or class cost which the driver education car is used. The fuel surplus fee is $2.00 per lesson, or per hour for any program which the driver education car is used.

Additional fees $______($______) – Possible additional fees for travel, additional trips, additional equipment, additional time, additional instructor, and other possible fees.

*All prices are listed to provide class, course, program, or package for Students for pick up/drop off with 30 minutes of offices, does not includes additional travel, time, equipment, additional instructor, or other factors which may change the price.

*Student has no guarantee to pass any class, course, or package offered by Driving School.

*Student under medical review with DMV must take road test at DMV.

*Driving School will provide Student, parent/guardian, or payee with price for services before beginning agreement unless both parties agree price can change, and possible change is stated in agreement.

Driving School is not a Virginia DMV licensed disabled driving school

All classes meet or exceed the Standards of Learning (SOL) for Virginia Department of Education (DOE), and the Virginia Department of Motor Vehicles (DMV). The minimum number of classes required to complete classroom instruction for students under 19 years of age is 36 class periods. The minimum number of periods required to license a student under 19 years of age is 7 behind-the-wheel, and 7 periods of in-car observation.

The State of Virginia requires The Student to obtain a proper learner's permit from the Virginia Department of Motor Vehicles (DMV). The Student may begin classroom instruction before obtaining a learners permit. However, The Student is required to obtain a valid learners permit, all required forms completed, and pay tuition in full prior to driving behind-the-wheel driving portion of the training. The Student MUST have their learner’s permit in their possession during all behind-the-wheel practical instruction.

Virginia Department of Motor Vehicles (DMV) does not require driver education training for students who are 19 years of age, or older student must hold learners permit at least 60 days to receive a driver license unless student completes a driver education in-class course, then student can receive driver license immediately upon passing all required DMV test.

The Driving School’s policy is to make available to The Student all required training within six (6) months from the date of this agreement being fully executed. Should The Student be unable to attend available training sessions offered the school is relieved of the aforementioned obligation.

Missing class, course, or lesson policy (VERY IMPORTANT)

If the Student must cancel a scheduled driving appointment, cancellation must be made before a minimum of 24 hours prior to the scheduled appointment. Driving School must be notified of an emergency situation before the scheduled class as early as possible. Because we offer convenience to all our Students, and for the concern of all our Students, should a Student not notify Driving School of an emergency, or be fifteen minutes late for any scheduled class the Student will be considered missing a class. A missed class must be made up at the next scheduled class, when the driving school can work the Student into the schedule. If a Student meets the definition of a missed class, the Student must wait until the next class for in-class, or when the Driving School can schedule a class at the Driving School’s convenience to help other Students receive their license on time, or the Student must pay a missed class fee. Should Student desire to make up a missed class the Student will be charged the missed class fee. The fee to make up a missed class is in an addition to the Package fee plus twenty-five dollar ($25.00) fee for each lesson. A lesson is a fifty (50) minute session which is conducted during the teaching session which the student missed. For Behind-the-Wheel a Student will miss two lessons (1-50 minute driving lesson, and 1-50 minute observation lesson), or two (2) lesson=fifty dollars ($50), a student which misses an in-class session which the Student wants to make up will be (2-50 minute lessons, or 4-50 minute lessons), or fifty dollars ($50) for two (2) lessons, or one hundred dollars ($100) for four (4) lessons. Should the person assigned to pick up the student be more than fifteen (15) minutes late, the Student will also be concerned the same as a missed class. This is due to the fact the Driving School has many other Student’s ready for class at a set time. If the pickup person is late, the other Student’s waiting will be late.

If The Student is unable to attend a scheduled class or scheduled behind-the-wheel driving instruction, The Driving School will make every reasonable effort to provide additional training classes, based on rotating classroom instruction scheduling or reschedule in-car instruction on a “first come first serve” basis. It is The Student’s responsibility to contact the Driving School to schedule make-up classroom instruction and in-car driving instruction. Additional fee will apply to any student which does not get prior approval from Driving School.

The Driving School reserves the right to deny The Student admittance to any class instruction or scheduled in-car driving instruction. If the Student is tardy the Driving School reserves the right to refuse admittance to the class. Should The Student be denied all agreements listed in document will apply.

The Driving School shall furnish a licensed instructor, classroom, teaching material, insurance, and a motor vehicle for instruction. The Driving School will provide all motor vehicle insurance for the purpose of in-car instruction.

Destruction of property, or the possession, distribution, or use of any tobacco product, alcohol, or use of drugs is strictly prohibited, and may, at The Driving School’s discretion; result in the cancellation of this agreement.

Should this agreement be canceled under such circumstances, all fees may be pro-rated, based upon hours of services provided prior to cancellation.

Payment of tuition must be received in full (cost of package, course, class, or program plus additional fees if fees apply) before instruction will begin. Payment will be received by Driving School in form of cash, credit card, debit card, check, certified check, or cahier check.

The Driving School does provide allowance for one-on-one, behind-the-wheel practical driving instruction and one-on-one in-class instruction. However, if student is under 18 years of age the parent/ legal guardian must give Driving School permission for one-on-one training.

Before a student can receive credit for in-car observation another student must be in the car behind-the-wheel driving. Student under 19 are required to have at least seven (7) in-car observation periods before student can pass the in-car portion of the class.

Students requiring classroom instruction can begin classes at Convenience Driving School classroom on ______/______/______, and classes will begin at ______am/pm. Convenience driving School will provide books, teaching material, and instructor. Student can bring items they want to ad in instruction, and students are required to attend all classes. If Student does not complete all classes, the Student must schedule a make date for class, or classes they have missed before receiving certification (all fees apply). Unless prior approval is given to the Student by Driving School to miss a scheduled Driver Education in-class and Driving School agrees to make up the missed class or classes, the Student must wait until the next scheduled class to complete any missed class or classes. Should Student not have prior approval from Driving School to make up missed class or classes, and Student wants to make up any missed Driver Education class, or classes, the Student must contact Driving School to schedule a makeup class or classes. The Student will be charged an additional $25 per lesson to make up class, or classes. (One (1) lesson is one-fifty (50) minute lesson).

The Driving School reserves the right to run specials, or advertisement programs which may offer a package, class, or course at a reduced price per this agreement.
