VOLUNTEER COUNSELOR AGREEMENT
2018 Summer Football Camps/Academies
Instructions: Please fill out the entire agreement, sign, and either mail OR fax it to Sports International. (If you are faxing this agreement in, make sure you use a black pen with dark ink to fill this form out.) Please check off the academy/camp(s) you are applying for. Please also provide us with an accurate e-mail that you check frequently as all volunteer information and confirmation packets will be sent to you this way. You should receive further notification concerning the status of your acceptance via email, approximately 4-6 weeks after it is received. If you applied at last year’s camp/academy or in the fall of 2016, we will begin sending out email confirmations starting February 2018.

We need your help in getting the word out regarding the camp to the players in the camper’s football program. The purpose of this email is to see if you could help us by passing out a camp brochure to each player in the camper’s football program (all ages). The only thing you would need to do is give the brochures to the coaches of each team so they could pass them out to the players at the end of practice. The brochures will offer each player in the players’ football program a $50 discount to camp.
