2018 High School Senior Award
Application Instructions
1. Graduating high school seniors or individuals completing their home schooling in the current school year, who are past or present members of the Mentor Soccer Club, are eligible to apply for this award. (A member is defined as an individual who has registered and played for a Mentor Soccer Club youth Rec, Travel, or Wave team).
2. Each recipient may receive this non-renewable award only one time,and will be offered withoutrestriction in regard to color, race, creed, sex, ornational origin.
3. The award is provided on condition that the funds be used toward first year tuition fees to your university, college, technical, nursing,or tertiary school.
4. The award will be apportioned to the recipient or recipients chosen by the Award Committee of the MentorSoccer Club. The maximum amount of the award to any one individual is $1,000.00.
5. Should the recipient find that for any reason he or she is unable to use the award during the ensuingschool year, the student forfeits all rights to it.
6. The criteria for giving this award shall include but not be limited to the following:
a. Mentor Soccer Club participation.
b. Applicant’s volunteer involvement with MSC.
c. Family members’ volunteer involvement with MSC.
d.High School Academic excellence and extra-curricular activity and community service participation.
7. Each applicant must submit a completed questionnaire. The questionnaire is available in two formats. The Word document is a “locked form” that can be filled out by “tabbing” through the text fields on your computer. Once complete, save it and print it out. If the applicant does not have access to Word, an Adobe .pdf version can be printed out and the applicant can fill it out by hand.
8. The applicant must complete the Mentor Soccer Club Award Application Questionnaire andreturn it, with the required attachments (see below), to their high school scholarship administrator byFriday, May11th, 2018. (MHSand LCHS, all others mail it in to Mentor Soccer Club – Scholarship committee)
Required Attachments
- Transcript of grades including class rank and total Grade Point Average (most recent grading period)verified by authorized signature.
- Home schooled applicants will submit equivalent records of academic achievement (SAT scores, GED, or similar, as well as a list of activities)
- Letter of Recommendation from a principal, guidance council,teacher,or civic/spiritual leader.
