Dear Adjudicator,

Enclosed is the Adjudicator Availability Form for our 2017 Spring Festivals. I ask that you return the completed form to me in the enclosed return envelope by February 1, 2017. You must return the form whether or not you intend to judge this year. Please do not delay in returning the form. If you are unsure of availability for specific dates, you can call, e-mail or write to me anytime up to five weeks prior to the festival dates with changes. If possible, I would prefer that you return the form to me by mail. If you plan to send it electronically, be sure to send me both sides of the form! Please note…due to phone line restrictions, I cannot receive faxed copies.

Cell Phone Number – If you use a cell phone, please provide the number and tell me which number (home or cell) you would prefer to have listed as your primary number for distribution to festival hosts (use check boxes).

Email – Please provide a current email address – check to see that the email indicated on the form is accurate.

Please indicate available dates by circling individual days on the calendar(s) printed on the back of your form. If you live or work in specific or nearby areas (Zones 2, 3, Long Island and NYC), your Availability Form will also include a place to indicate availability for Mid-Week festivals. Also included in your packet is a listing of Mid-week festivals (yellow for Zone 2/3 and green for Long Island/NYC). If you are a Majors adjudicator, your Availability Form will include a place to indicate availability for majors. Also included in your packet is a listing of Majors festivals (blue). If you feel you did not receive the correct forms, contact me immediately.

All availability (weekends, mid-week, and majors) must be recorded on ONE Availability Form. Please do not return the other lists – keep them for your own records.

Please check carefully each piece of data that is preprinted on your form. If data is incorrect or missing, please correct or add it using a different colored ink or by highlighting the changes.

School/Work Information: Please be sure we have your current school information listed.

Exceptions:

·  If you are retired from teaching, your School/Business field should read “Retired.”

·  If you are retired from teaching and have, like many, taken another position, please indicate your business’s name and phone number, including area code, so that you may be reached during the day if necessary.

Availability: Please check only one box. Please note that Adjudicators who are consistently unavailable may be removed from the “Active” list. This simply means that you would not automatically receive an Availability Form each year. To be “Reactivated” you would need go online to the Assignor of Adjudicators page at www.nyssma.org, download a blank form, and return it to the assignor indicating your availability.

On the reverse of the Availability Form are the festival weekend dates (in bold).

·  Please circle each day you are available – if you are available only one of the two days on a weekend, please circle only that day which you are available.

·  I have provided a list of Weekend Solo & Ensemble Festivals on the back of this sheet. Please use Festival Numbers when referring to specific festivals.

Notate Restrictions: If you have a maximum number of assignments you are willing to take, please do not limit your availability to that number of weekends. Please indicate all weekend dates you are available and indicate, in the appropriate space, your maximum number of assignments and I will honor your request. By doing so, it will help me to assign you when and where you are most needed. Likewise, if you have restrictions on travel distance or locations make sure you let me know in the space provided. If you were available to judge last year but were not assigned, please indicate that in the “other restrictions” space. I will make every effort to see that you are assigned this season.

Your Signature: Your signature indicates your commitment to adjudicate for NYSSMA on the dates you are available and a further commitment to hold those dates available until at least two (2) weeks prior to the festival dates.

Honoraria, Lodging, and expenses:

Honoraria: All-State/Majors = $25.00; Regular = $20.00; Local = $16.00

Lodging: NYSSMA will pay for the cost of a single room for adjudicators who live too far from the festival site to stay at home. Festival Chairpersons will make the necessary arrangements for housing for judges.

Travel: Mileage for 2017 will be reimbursed at $.54 per mile. NYSSMA® adjudicators will be reimbursed for mileage and tolls incurred when driving within New York State. Adjudicators are expected to take the shortest route that is reasonable for the traffic and/or weather conditions. If the adjudicator lives outside of the state (New York State), NYSSMA® will reimburse up to a total of 150 miles (75 miles, each way) beyond the New York State border in addition to the travel within New York State. Receipts for tolls, etc., should be submitted to the Zone Representative with your expense form. Travel by plane, limo or taxi requires prior approval from the Assignor of Adjudicators. NYSSMA does not reimburse for car rentals.

Meals: For weekend festivals: Friday dinner and Saturday lunch will be provided on-site FREE. For midweek and in house festivals, dinner will be provided on-site FREE. In addition, a meal allowance (1 day = 2 session) will be given to all adjudicators.
