Please complete the enclosed forms for EACH Junior Chef for this year’s Summer School Program. Please note the time of class is as follows:

Location: 5224 South Dale Mabry Hwy, Tampa, Florida 33611
Time: 10:00 am – 12 noon & 1pm – 3pm*
JCOA Kitchen Rules Are As Follows:
❖ Be Prepared! Always come to class ready to cook
❖ Wear closed-toed shoes (like tennis shoes)
❖ Wear Your Junior Chef’s Apron Daily (Clean if necessary)
❖ Have your hair pulled back
❖ Wash your hands with warm water and soap when you come in..and have a seat, ready to cook
❖ Always wash hands after handling raw eggs and meat
❖ When tasting, use your tasting utensils ONLY and put in sink immediately
❖ Never Touch Microwave or Hot Plates unless standing with your JCOA Chef Instructor
❖ All appliances and equipment are to be used with JCOA teacher permission and supervision
❖ Be friendly by sharing tasks with each other.
❖ Listen to your JCOA Education Chef/Teacher at all times
❖ Practice Safety when using all utensils
❖ NO HORESEPLAY

Release of Liability Form:
The undersigned individual desires to use one or more of the Junior Chef’s of America’s facilities (collectively the “Facility”) and/or to participate in activities or programs (collectively the “Activities”) sponsored by or involving Junior Chef’s of America, Inc., Ybor City Food Tours, Inc, and all franchise’s thereof (collectively, “JCOA”). In consideration for Junior Chef’s Of America, Inc. permitting me to use the Facility and permitting me to participate in the Activities, I have agreed to execute this Release of Liability (the “Release”).
I acknowledge that using the Facility and participating in the Activities may involve certain inherent risks, including the risk of personal injury. I agree to assume all such risks, as well as any other risks involved in sing the Facility, participating in the Activities or participating in any other activity sponsored by or involving Junior Chef’s of America, Inc. I release and discharge Junior Chef’s of America, Inc and al of its employees, agents, representatives, partners, and members, as well as all other persons or entitles that might have any liability whatsoever to me related to the Facility, as well as any and all other persons or entities that might have any liability whatsoever to me related to the Facility, the Activities, or Junior Chef’s of America, Inc.. This Release is intended to release and discharge the Released Parties from all damages, actions, claims and liabilities of any nature, specifically including, but not limited to, damages, actions, claims and liabilities arising from or related to the negligence of the Released Parties. I further agree to indemnify, hold harmless, and defend Junior Chef’s of America, Inc and each of the other Released Parties from and against any loss, damage, liability, and expense, including costs and attorney’s fees, incurred by Junior Chef’s of America, Inc or any of the other Released Parties as a result of my using the Facility, participating in the Activities, or participating in any other activity sponsored by or involving Junior Chef’s of America, Inc.
Junior Chef’s of America, Inc reserves the right to use any photograph taken at the Facility, or in connection with any Activities or other activity involving Junior Chef’s of America, Inc., including but not limited to those taken during a program, a birthday party, cook-off, a group event, parent informational meetings, or other Activity, to be used in Junior Chef’s of America, Inc’s promotional materials, brochures and web-site.
THIS RELEASE IS A BINDING LEGAL CONTRACT, PLEASE READ IT CAREFULLY BEFORE SIGNING.
TO BE SIGNED IF PARTICIPANT IS A MINOR
I represent that I am the parent or legal guardian of the above individual and hereby consent to the individual using the Facility, participating in the Activities, and participating in other activities sponsored by Junior Chef’s of America, Inc. In consideration for Junior Chef’s of America’s allowing the above individual to use the Facility, participate in the Activities, and participate in the other activities. I agree, personally and on behalf of the individual, to be bound by the terms and conditions of this Release. I further agree to indemnify, hold harmless and defend Junior Chef’s of America, Inc. and all other Released Parties from and against any loss, damage, liability, and expense, including costs and attorney’s fees, incurred by Junior Chef’s of America, Inc. or the other Released Parties as a result of the above individual using the Facility, participating in the Activities or participating in any other activity involving Junior Chef’s of America, Inc.
