HEALTH REIMBURSEMENT ARRANGEMENT PLAN (for Employees enrolled in GHT):

- Single Plan = $400 per year
- Employee + 1 = $800 per year
- Family Plan = $800 per year
SECTION 125 PLAN:Superior State Administration
- Pre-tax payroll deduction for employee share of insurance premiums, childcare and medical out of pocket costs.
- Annual max allowed - $5,000 for childcare and $2,600 for healthcare. Up to $500 can be rolled over to the next year.
RETIREMENT: Wisconsin Retirement System (WRS) Fund - Wisconsin ETF

- Pays up to 75% of gross salary to a max of $4000 per month
- For mental or physical disabilities
- County pays for the 90 day waiting program
- Employee may pay to lower waiting period to 60 or 30 days.
- Note: for 2017 – premium holiday (state not charging for coverage)
- Eligible – 1st of the month after 6 months
LIFE INSURANCE:Wisconsin Public Employers Group Life Insurance – Wisconsin ETF

- After seven (7) years of employment - three (3) weeks;
- After fifteen (15) years of employment - four (4) weeks;
- After twenty-five (25) years of employment - five (5) weeks;
- After twenty-six (26) years of employment - five (5) weeks and one (1) day;
- After twenty-seven (27) years of employment - five (5) weeks and two (2) days;
- After twenty-eight (28) years of employment - five (5) weeks and three (3) days;
- After twenty-nine (29) years of employment - five (5) weeks and four (4) days;
- After thirty (30) years of employment - six (6) weeks.
PERSONAL DAYS:2 per year
SICK LEAVE:1 day per month, maximum 90 days
HOURS PER WEEK:37.5 to 40 hrs - varies by position.
CALL IN PAY:$200 to $300 per week while on call – varies by position
HRS:May vary by department.
