2017 - 2018 Title I, Part D CRP Application Guide (DOC)
Application Details1. For each facility that is implementing a Title I program, please provide a complete description of activities to be supported with Title I funds. The response should address:
a) the process used to assess the educational needs of the students, and the results of the comprehensive needs assessment;
b) the instructional program, pupil services, and procedures that will be implemented to meet identified student needs;
c) the type, content, and duration of professional development supported with Title I funds for teachers and staff; and
d) the coordination of state and federal programs to support the educational needs of students.
The response should address the following elements for programs at each facility:o Describes the procedures used to assess the educational needs of the students, including the identification of specific assessments.
o Describes the educational needs of students to be served based on assessment results.
o Describes the instructional program, and pupil services that will be supported with Title I funds to meet the educational needs of students. For each program component (e.g. instructional services, transition services), the description should include:
§ program description;
§ number of students participating;
§ Title I funded staff providing the service;
§ length of program;
§ anticipated outcomes; and
§ how progress will be measured.
o Describes the type, content and duration of Title I funded professional development that will be provided to teachers and staff that serve Title I students.
o Describes how the program will be coordinated with other appropriate State and Federal programs, vocational and technical education programs, State and local dropout prevention programs, and special education programs.
2. Describe how the State Agency will coordinate with local education agencies or alternative education programs attended by incarcerated youth prior to their incarceration to ensure that student assessments and appropriate academic records are shared.
The response should address the following elements:o Identifies the person at the agency responsible for coordinating with local education agencies or alternative education programs.
o Describes the process for consulting with local education agencies or alternative education programs to ensure that student assessments and appropriate academic records are shared.
3. Describe the State Agency’s procedure for ensuring that in making services available, priority is given to children and youth who are likely to complete incarceration within a 2-year period.
The response should address the following elements:o Identifies the person at the agency responsible for ensuring that priority is given to children and youth who are likely to complete incarceration within a 2-year period.
o Identifies the criteria used to determine student eligibility and determine priority.
o Describes how the agency ensures that priority is given to children and youth who are likely to complete incarceration within a 2-year period.
4. State agencies are required to reserve a minimum of 15%, and no more than 30% of their Title I allocation for transition activities. Please specify the amount reserved for transition activities and describe the programs/activities available to assist youth in their transition as they re-enter school, enroll in postsecondary education program or a career and technical education program, or as they seek to ensure employment. Indicate how these services are above and beyond what is ordinarily provided.

The response should address the following elements:o Explains how the agency determines which transition services will be available to children and youth.
o Describes how the agency identifies eligible students.
o Describes the programs/activities available to assist children and youth in their transition to:
§ re-enter school;
§ enroll in a postsecondary education program;
§ enroll in a career and technical education program; or
§ seek to secure employment.
5. Please provide the name, title, address, telephone number and email address of the individuals responsible for issues relating to the transition of children and youth.
The response should address the following elements for each facility:o Provide the name, title, address, telephone number and email address of the individual that is responsible for issues relating to the transition of children and youth by facility.
6. Describe the institution’s efforts to coordinate with businesses for training and mentoring for participating children and youth.
The response should address the following elements:o Identifies businesses or local organizations that the agency is coordinating or seeking to coordinate with for training and mentoring.
o Describes the capacity of the business or local organization in providing training and mentoring for participating children and youth.
7. Title I Part D requires an evaluation of the program conducted under this part, with data on participation disaggregated by gender, race, ethnicity, and age while protecting individual student privacy. This is done to determine the program’s effect on the ability of participants to attain the following outcomes:
a) Maintain and improve educational achievement.
b) Accrue school credits that meet State requirements for grade promotion and high school graduation.
c) Make the transition to a regular program or other education program operated by an LEA.
d) Complete high school (or a GED) and obtain employment after leaving the correctional facility or institutions for neglected or delinquent children and youth.
e) As appropriate, to participate in postsecondary education and job training programs.
(A) Please describe the evaluation results for 2016-2017 school year, and how these results informed the planning and improvement of the Title I program for the upcoming year.
(B) Describe the Department of Correction’s plan for evaluating the 2017-2018 Title I, Part D program(s).
The response should address the following elements:o Describes any preliminary or final evaluation data results (outcomes) for the 2015-2016 2016-2017 program’s effect on the ability of participants to attain the above outcomes (a – e).
o Describes how the results of the most recent evaluation (2016-2017) were used to plan and improve the 2015-2016 programs.
o Describes how the agency will evaluate the 2017-2018 Part D program’s effect on the ability of participants to attain the above outcomes.
o Identifies the data and information that will be used to evaluate the 2017-2018 Part D program’s effect on the ability of participants to attain the above outcomes.
