2016 Kravitz Orthodontics Scholarship for Academic Achievement

Kravitz Orthodontics is proud to offer scholarships to graduating seniors planning to enter and attend a university, college or technical school in the fall following graduation. The PTSA Scholarship Review Committee will choose applicants on behalf of Kravitz Orthodontics based on eligibility requirements and completion of this application.

Members of the review committee shall remain anonymous. The committee will read and score all applications and essays resulting in the selection of one candidate as recipient of the Kravitz Orthodontics Scholarship for Academic Achievement. One scholarship will be awarded in the amount of $400.

The scholarship funds must be utilized for school expenses only. The PTSA will prepare scholarship checks written directly to the institution. If a recipient fails to complete his or her obligations as outlined, the unused scholarship allocations and/or forfeited funds will remain the property of Kravitz Orthodontics.

Completed Scholarship Applications must be submitted to Mrs. Porto in the Career Center by May 2, 2016.

Eligibility Requirements:

·  Must have been a JCHS student for at least 2 years

·  Must have a GPA of 3.0 or higher

·  Student must be a member of the JCHS PTSA

·  Must be accepted to a degree or certification program at an accredited junior or community college, technical institute, or four year college or university (to be verified by JCHS school counselor)

Application Instructions

DO NOT STAPLE the pages of your application. Use a paper clip. Include the following pages:

1.  Cover Sheet

2.  Unofficial Transcript

Overview: Minimum GPA of 3.0. Your transcript should show academic success or academic progress in

challenging classes.

Detail: An unofficial transcript including senior year grades from 1st, 2nd and 3rd quarter must be attached.

3.  Record of Activities

Overview: Show that you had a high level of involvement in community or school activities. There should be strong evidence of leadership, personal responsibility and initiative, with consistent and dedicated participation in activities over the high school years.

Detail: Include a year by year listing of activities, limited to the high school time frame only-- positions held,

any honors or awards you received, time commitment (total hours), etc. Activities may be school related, community related, paid or unpaid. Remember to include all volunteer work and any camps or academic enrichment programs attended. You can use your resume, if it includes sufficient information, as this attachment.

4.  Essay

Tell us something that you want us to know about yourself that is not elsewhere in the application. Explain how this has contributed to you to be the person you are today.

Detail: Essay should be no more than 500 words.

5.  You must also obtain Two Letters of Recommendation

Overview: Two letters of recommendation from non-related adults are required. One letter must be from a member of the JCHS community (teacher, staff, coach, etc.) The second letter should be from another adult who knows you well but does not have to be associated with JCHS.

Detail: Fill in your name at the top of the Letter of Recommendation Form and give it to your reference as

soon as possible. Remind the person who is writing the letter of recommendation that it is due to Mrs. Porto by

May 2, 2016 and that the Letter of Recommendation Form must accompany the letter.

The Kravitz Orthodontics Scholarship for Academic Achievement is a one-time, $400 award given to a graduating senior who has been a John Champe High School student for at least 2 years, has a GPA of 3.0 or higher, and has been accepted to a degree or certification program at an accredited junior or community college, technical institute, or four year college or university.

Please complete this form and submit it with a letter of recommendation that addresses the student’s character, academic ability, and personal commitment to continuing education. A significant factor for the award of the scholarship is the applicant’s involvement in community or school activities. Please include comments on such characteristics as leadership, initiative, perseverance, integrity and responsibility, as well as any additional information or extenuating circumstances that you believe might help the committee make its decision.

The deadline for receipt of this form and the letter of recommendation is May 2, 2016. Please send it prior to May 2 to:
