The purpose of this evaluation is to help the school leadership team determine if the targeted assistance program is effective in increasing student achievement. Part of this process is an annual review of the strategies in the targeted assistance Title I plan to determine if they are contributing to the desired outcomes either in terms of improvement in student achievement or increases in other activities that lead to student achievement. Please complete this form for the students who received targeted assistance services. Attach the list of all students who received Title I targeted assistance services at any time during the 2016-2017 school year.
