2016-2017AFTERSCHOOL ENRICHMENT PROGRAM

Quality afterschool care is offered Monday through Friday during the entire school year on regular school days, early release days, and most no-school days. If you sign up for the whole year, we consider you a full time student.

The yearly full-time tuition can be divided and paid in 10 equal monthly payments. Monthly payments are the same each month from September 2016 to June 2017 regardless of attendance, holidays, family emergencies, and school closures due to weather or power outages.

Full-time tuition includes pick-up service from school, snack, daily homework help, Chinese and math tutoring, and as many Chinese classes as they can attend.

Before school care, holiday & school break care are offered at additional cost. Before school care can be arranged on an individual basis, holiday & school break might be canceled if there are less than 5 students in the care.

If you don’t sign up for the whole year but wish to have afterschool care occasionally, there are three options for you. Transportation from the students’ school to ours is offered for you at $5 per student per pick-up, they can enjoy a snack, homework help, Chinese and math tutoring, and Chinese classes on the days you are here for afterschool care.

I UNDERSTAND AND AGREE TO THE FOLLOWING TERMS:

  1. There will be a non-refundable annual insurance fee of $50 per family.
  1. The yearly tuition can be divided and paid in 10 equal monthly payments. Monthly payments are the same each month from Sept. 2016 to June 2017 regardless of attendance, holidays, family emergencies, and school closures due to weather or power outages.
  1. Tuition includes pick-up service from school, snack, daily homework help, Chinese and math tutoring, and as many Chinese classes as they can attend. There are additional charges for other enrichment classes.
  1. “Ea. add. sibling” refers to each additional child from the same family.
  1. A last month deposit is due at the time of registration. This deposit is NON-REFUNDABLE if my child withdraws from the program before the end of the school year.
  1. Monthly tuition is due on the first school day of the month, and a $20 late fee will be charged on payments received after the 5th of the month.
  1. There will be a $20 charge on each returned check.
  1. If my child is not picked up by 6:30 pm, I WILL be charged a $5.00 late fee for the first 15 minutes and $1.00 per minute thereafter.
  1. All students are expected not to touch or disrupt any of the belongings of the host church. I am expected to pay for any item (whether it belongs to SCA or CCOJ) that my child damages or breaks.
  1. It is my responsibility to make sure that Sammamish Chinese School has my updated e-mail address on file.

AFTERSCHOOL PROGRAM CONSENT FORM

I hereby give my permission and consent for my childto be picked up at

(School name) and sent to Sammamish Chinese School by the teachers of Sammamish Chinese SCHOOL. I understand that the teachers from

Sammamish Chinese School will take every precaution and care to insure my child’s safety. I hereby release Sammamish Chinese School from any liability and from any and all claims against them, individually or collectively, for any injuries which might be received while traveling to Sammamish Chinese School.
