2016-2017 MPI-HAC Board of Directors Nomination Form

Thank you for taking time to nominate one of your peers for a position on the MPI-HAC Board of Directors.

You may nominate as many people as you choose; however, only one nomination per form. Therefore, you will have to revisit the nominations process for every person and position.

After you submit your nomination, your nominee will receive an email congratulating them and they will be asked to complete the Board Candidate Application. If they do not complete their submission form by January 15, 2016 they will not be considered for a board position.

Board members must be a current member of MPI-HAC to serve on the board.
