Put a check mark next to “Dept Check in the Works” in the Totals box, total up your guest’s fee. Mail the second page, containing the Receipt, to your employer for payment and mail this formto Terry Ball with your personal check to pay for your guest’s meals.
Make sure you circle “Yes” or “No” to indicate if you are attending each free meal. The Hotel requires an accurate meal count from us.
If you are paying for both the seminar fee and your guests yourself, add the appropriate fee for the seminar in the “Seminar Fee” section of the “Total” column, total the additional fees for your guests, and mail this form only to Terry Ball with your personal check. You may keep the second page for your records.

The 2010Summer Seminar will be held at the Red Lion in Pendleton, Oregon. We have blocked off 60 rooms for our use until June 1, 2010. Make your reservations before then, or you will lose this rate. Each attendee is responsible for making their own reservations at the hotel. Make sure you identify yourself as an NPEA member to receive the low negotiated rates.
