TIME: FROM 6:00 P.M. TO 8:00 P.M.
WHERE: BEHIND BOZEMAN HIGH SCHOOL GAME FIELD
COST: Pre registration fee of $40.00 (Before July 1st)
Registration fee at the door $45.00 (July 18th)
Make sure you have your physical for 2016-2017 school year completed
($5.00 DISCOUNT FOR EACH ADDITIONAL CHILD IN YOUR FAMILY)
EACH PARTICIPANT WILL RECEIVE A CAMP T-SHIRT

I hereby authorize the directors of the BOZEMAN MIDDLE SCHOOL FOOTBALL CAMP to act for me according to the best judgment in any emergency requiring medical attention. I hereby waive and release BOZEMAN FOOTBALL CAMP AND BOZEMAN HIGH SCHOOL. I know of no mental or physical problems which might affect my child’s ability to safely participate in this camp. I will be responsible for any medical or other charges in connection with his attendance at camp. I have read, understand, and agree with the conditions of this registration.
