2015 Southern MN AFP Awards for Philanthropy

CALL FOR NOMINATIONS

The Southern Minnesota Chapter of the Association ofFundraising Professionals invites nominations for the 2015 AFP Awards for Philanthropy.

Individuals, corporations, foundations, or other organizations may be nominated by any member of the public. Nominators are welcome to submit nominations for all categories. Nominees not selected for recognition will automatically be reconsidered the following year.

Posthumous nominations will not be accepted. However, if a nominee passes away after the nomination is received, the nomination will be considered.

Deadline for submission is September 11, 2015.

Nomination materials should consist of:

A cover letter

A completed nomination form (see last page)

Narrative information which addresses the selection criteria for the award, and is a maximum of 3 typed pages in no smaller than 12 point type

A maximum of 3 letters of endorsement

Resumes, newspaper clippings, annual reports, etc. will NOT be accepted as substitutes for narrative responses to the selection criteria, and should not be submitted as part of the nomination

All nomination materials shall remain the property of the Chapter

THE SELECTION PROCESS

Nominations will be reviewed by civic leaders from Southern Minnesota and members of the AFP Southern Minnesota Chapter Board of Directors. Awards may not be given in every category each year.

All award recipients will be recognized during the Philanthropy Day Program on November 10, 2015 in Rochester.

2015 AFP Awards for Philanthropy

SELECTION CRITERIA(Criteria listed in the order of importance)

Outstanding Philanthropist

This award is presented to an individual or family with a proven record of exceptional generosity. Recipients will have demonstrated outstanding civic and charitable responsibility and generosity, which encourage others to take philanthropic leadership roles on a community, national and/or international level. Your nomination should include the following information with specific examples:

  1. Evidence of direct gift support
  2. Description of impact of gift support
  3. Evidence of encouragement and motivation given to others to take leadership roles in philanthropy
  4. Description of participation in gift support of other nonprofit organizations in the community, nationally and/or internationally
  5. Evidence of philanthropic support of innovative approaches to solving problems

Outstanding Philanthropic Organization

The organization must have demonstrated outstanding commitment through financial support and through encouragement and motivation of others to take leadership roles toward philanthropy and community involvement. List your nominee's achievements, including:

  1. Evidence of direct gift support indicated by support of projects in the community, nationally and/or internationally
  2. Description of impact of gift support on projects
  3. Description of specific achievements accomplished through the organization's efforts
  4. Evidence of encouragement and motivation given to others to take leadership roles toward philanthropy and community involvement
  5. Evidence of philanthropic support of innovative approaches to solving problems

Outstanding Volunteer Fundraiser

The individual eligible for this nomination must have demonstrated exceptional leadership as a volunteer for a major fundraising project for more than one nonprofit organization. The recipients(s) should demonstrate exceptional skill coordinating and motivating groups of volunteers for fundraising projects for the benefit of charitable institutions, and a commitment to the advancement of philanthropy. List your nominee's specific achievements for the past year of several years, such as:

  1. Evidence of quality and organizational ability in fundraising
  2. Description of time, effort and financial commitment
  3. Description of fundraising goals achieved by his/her effort
  4. Evidence of success in recruiting and motivating others
  5. Description of participation in other local, national and /or international nonprofit organizations

Outstanding Youth Award

The youth award is given to an individual or group of individuals that has shown outstanding philanthropic effort within their community. Traditionally, this award is given to an individual or group high school age or younger. Those receiving this award have both grown through their experience and have provided a benefit to the community they serve. Please provide the following information in support of your nomination for this award:

  1. Background on the individual or group being nominated
  2. Description of philanthropic effort undertaken
  3. Results of this effort
  4. Who or what was helped through this effort
  5. What the individual or group learned from this effort
