1. I will treat this animal as a companion and family pet, giving it love, care and attention.

2. This dog’s unknown background and medical history has been discussed with me.

3. I will feed this pet daily and will provide a continuous supply of fresh, clean water.

4. This pet will live in my home and will not be isolated from the family or kept in a crate. I will not let this pet roam free, or keep it in a crate, pen, garage, or on a tether. I will walk the dog on a leash or it will be exercised in a fenced area. It will not be left alone in the yard when the family is not home.

5. Choke collars or aversive training methods must not be used.

6. I agree to provide all future medical care. I also agree to seek prompt medical treatment for illnesses or injuries by a licensed Veterinarian.

7. I agree to observe all local animal ordinances.

8. I certify this dog will be my own personal pet and will not give it to another party as a gift.

9. I agree I will not sell, re-home or abandon this dog at a shelter or pound at any point in this dog’s life.

10. I agree if, at any time, I cannot keep this pet, for whatever reason, I will return the dog identified above to PVCA Animal Rescue Society or a volunteer of PVCA Animal Rescue Society.

11. I agree to accept possession and responsibility for the care of the dog including all financial responsibilities associated with such care.

12. I will not crate train the pet nor ever crate the pet except for safety in travel. If there are reports that the pet is being crated it will be immediately surrendered to pvca or its representative.

13. I agree to allow a representative of PVCA Animal Rescue Society access to the animal upon request including home visits and veterinarian records.

14. I further understand that PVCA will always remain the owner of the pet and failure to comply with the terms and conditions of this contract will result in forfeiture of the animal identified herein to PVCA Animal Rescue Society without a refund of the fee.
