Dear parent(s):

It is with great pleasure that I welcome your child to my class. We can all look forward to a very exciting and rewarding school year. In order to provide my students with an excellent educational climate, I have developed the following Classroom Discipline Plan that will be in effect at all times.

Rules:

1. Turn in all assignments on time.

2. Have all supplies needed as you enter the class.

3. No cell phone visible or turned on – unless specified by teacher.

4. Respect yourself and others.

5. Always try your best and seek clarification when needed.

If a student breaks a rule:

1st time: Verbal warning

2nd time: Student, teacher conference

3rd time: Phone call or email to parents

4th time: Parent, student, teacher conference

5th time: Office Referral

Severe Disruption: Immediate office referral

Any Rock throwing/ fence climbing may result in serious consequences from school.

Students who behave appropriately will be positively rewarded with :

Positive recognition, treats

It is in your child’s best interest that we work together with regard to his/her education. I will keep you informed about your child’s progress in my class.

I have already discussed this plan with your child, but would appreciate it if you would review it with him/her before signing and returning the form below.
