Summary Narrative

Each Nomination Form must be accompanied by a summary narrative that describes the housing development, program or organization nominated for an award. The following items must be addressed in the narrative:

  1. Describe the housing development, program, organization, preservation/revitalization or conservation effort including funding strategies, planning processes and partnerships. Explain all unique or innovative aspects.
  2. Who were the persons or organizations involved in implementation?
  3. What were the expected outcomes?
  4. What were or are the results? Describe how the actual results relate to the expected or planned outcomes. Describe the impact of this project.
  5. Describe the beneficiaries of the housing development, program, organization and preservation/revitalization or conservation effort. Please quantify whenever possible. (Number of individuals or families, income level, special characteristics, etc.)

Electronic submissions are preferred, but mailed hard copies will be accepted. No more than six pages of narrative may be submitted regardless of submission format. A 12-point font must be used. Up to 10 items of supporting materials such as program literature, news clippings, data, letters of support and photographs or slides may be attached or enclosed. We plan to have a Power Point presentation on the award winners, so please include photographs or slides of your nomination.

All submissions, including electronic submissions, must be received by 5 p.m. on Friday, Sept. 9, 2016. Please note that waiting until shortly before 5 p.m. to submit an electronic application could result in an email failure. If submitting hard copies, please submit one unbound original and two unbound copies. These materials will not be returned.

Nomination packages (electronic or hard copy) submitted past the deadline, without the appropriate number of copies or exceeding the page limits may be determined ineligible for consideration. Please submit all materials in the same format, either electronic submission or hard copy. PLEASE NOTE: THE NOMINATION FORM MUST BE SUBMITTED IN MICROSOFT WORD.
