PCP SELECTION LETTER
Dear Participant:
As a requirement of the Trinity Welfare Benefit Plan, by January 1, 2018, you will need to select a Primary Care Physician (PCP), providing care to each covered participant. The PCP must be a Practitioner who practices in General Medicine, Internal Medicine, Family Practice or Pediatrics.
The Health Choices portal is a powerful and confidential source for easy access to useful participant information. Some of the features available to you through My eHealth Choices include claims status inquiries, review of your provider directory and print I.D. cards.
To access My eHealth Choices to select your Primary Care Physician, go to www.preferredhealthchoices.com, click on “Participants” button, and click on “Sign up” to register, if you are a first time user. Use the attached instructions to select or enter your PCP selection/s.
