NOTE: With additional expenditures in the current grant FY 2017-2018 for the new Youth awards, increases to maximum encumbrances for the Transition Grant and the ILC Tier Augmentations, the Title VII-B carry-forward amount is currently estimated to be at $1.5 Million.

11/1/2017 Announcement of revised Transition grant from Victor Duron

Greetings IL Leaders;

I am pleased to announce that, with the support and leadership of the SILC, we will be making the following changes to the IL Transition Grant to enhance the IL network’s ability to support consumers with transition services;

  • The total transition grant fund will be increased from $150,000 to $200,000
  • The maximum allowable expenditures per consumer will be increased from $4000 to $4500
  • The grant provision language has been changed as follows to increase flexibility in use of the funds (language changes are highlighted and marked in between brackets)
  • Transition funding is available to assist with a consumer’s transition from one of the following types of institutional facilities: licensed skilled nursing facility, intermediate care facility for the developmentally disabled, state Hospital for the mentally ill, developmental center, or a California veterans home.The facility must be named in the application for funding. Transition funding may NOT be used for consumers who have already transitioned out of one of the above-identified facilities. [In addition, transition funding may be used to assist with a consumer’s transition from a more restricted setting, such as an emergency shelter, temporary housing, or hospitals, into the community setting of their choice. Transition funding may be used for consumers who have already transitioned out of a more restricted setting, other than those institutional facilities identified above, with preapproval from the DOR.]

In addition, in an effort to streamline and centralize DOR processing of transition grant request, IL Unit Manager Carrie England will now be the lead for processing all Transition fund requests. Carrie will be reaching out shortly with additional guidance on submitting transition fund requests and reporting for the activities now allowable per the updated grant language.
