2. What special insurance and securities licenses do you hold? When did you receive them and from whom?

3. What types of insurance products to you sell? Commercial or personal lines? Please list all insurance products.

4. Do you offer complete financial planning services to your clients? If so, what types of security and investment products to you sell? To commercial accounts or individuals? Please list all products.

5. Are you responsible for sales, underwriting and/or adjusting? Describe your responsibilities in detail, along with the number of accounts that you manage or the region for which you are responsible.

6. If you're in sales, do you focus primarily on servicing existing clients or are you involved in new business development? Have you increased your region and/or account base? By what percentage? What is the annual premium volume that you generate?

9. What have been your achievements in this position? Have you increased premium volume, reduced claims paid, prevented fraud, improved agency earnings, exceeded sales goals and premium objectives, or anything else quantifiable? How do you rank in your organization and/or region?

10. Do you have supervisory, administrative and/or leadership experience? At what level do you function within the organization? Please describe your management responsibilities in detail.

11. What are your reporting and administrative responsibilities? Do you have an administrative support person?

12. Are you responsible for recruiting and hiring new personnel for your organization? Did you bring in any people that are now top producers?

13. Do you have budgeting, accounting or any other type of financial responsibilities for the organization? Any specific contributions to the bottom-line financial performance of the organization?

14. Have you been involved in any high-profile cases? Did they receive media coverage?

15. Do you work with third-party administrators, self-insured agencies, insurance companies, municipalities, general agents or captive agents? Any others? For what reasons/activities and how frequently? Any notable results?

16. Does your position/your career require any specialized regulatory and policy knowledge? Describe in detail.

17. Have you ever participated in the development and/or market launch of any new insurance products? Please provide detail regarding the product, market, launch and financial performance.

18. Do you belong to any professional or community associations? Any leadership or financial roles?

19. Are you active in continuing education? Please list courses, who offered them and dates.
