1  OSHA Injury/Illness Recording and Reporting

General

The Department shall comply with OSHA regulations for recording and reporting occupational injuries and illnesses as prescribed in 29 CFR 1904. This requires some injuries and illnesses to be recorded on the OSHA 300 Log of Work-Related Injuries and Illnesses and the OSHA 301 Injury and Illness Incident Report.

Responsibility

The Manager Base Aviation Maintenance at each base shall maintain appropriate and current logs and follow OSHA required posting procedures. The posting of the OSHA 300A Summary of Work-Related Injuries and Illnesses is the responsibility of the [position title]. The [position title] shall forward a copy of the previous year OSHA 300 log to the Safety Manager/Officer by February 1 of each year.

Determining OSHA Recordability

OSHA recordable injuries and illnesses can be determined by asking the following questions:

1.  Is this incident an occupational injury or illness (caused by the work or work environment)?

2.  Does the incident fall within any of the following?:

Illness which is diagnosed

Injury which results in any of the following:

a.  Medical treatment other than first aid

b.  Loss of consciousness

c.  Restriction of work or motion

d.  Transfer to another job

e.  Death

Both of the above conditions must be met for the incident to be OSHA recordable. Each recordable case shall be entered on the log within seven (7) workdays of receiving the report.

Reporting Fatalities or Hospitalizations

A written or oral report must be filed with OSHA within eight (8) hours of any incident resulting in:

a.  A fatality to one or more employees, or

b.  The hospitalization of three or more employees from a single incident

Reports may be made by telephone or in person to the nearest Area Director of the U.S. Department of Labor, Occupational Safety and Health Administration, or to the 24-hour OSHA Emergency Hotline, 1-800-321-OSHA (6742).

Retention of OSHA Records

The OSHA 300 Log of Work-Related Injuries and Illnesses and the OSHA 300A Summary of Work-Related Injuries and Illnesses must be retained for five (5) calendar years and must be available for inspection.

Posting Requirements

A copy of the OSHA 300A Summary of Work-Related Injuries and Illnesses for the previous completed year must be conspicuously posted in the facility no later than February 1 and must remain in place until April 30.
